研究目的：

基于整合照护模式下养老机构失能老人长期照护干预方案的构建已经完成,为了验证养老机构失能老人长期照护方案的可行性、适宜性及有效性,需要在养老机构开展干预方案的实施研究进行实证研究。  

Objectives of Study：

The construction of a long-term care intervention programme for the disabled elderly in nursing homes based on the integrated care model has been completed, and in order to verify the feasibility, appropriateness and effectiveness of the long-term care programme for the disabled elderly in nursing homes, it is necessary to carry out an empirical study on the implementation of the intervention programme in nursing homes.

纳入标准：

1.年龄≥60岁 , 2.符合《老年人能力评估规范》GB/T42195-2022内相关评定标准， 3.知情同意, 自愿参与本研究。

Inclusion criteria

1.Age >= 60 years; 2. In line with the relevant assessment standards in the "Standard for the Assessment of the Ability of the Elderly" GB/T42195-2022, 3.Informed consent and voluntary participation in this study.

排除标准：

1.有严重精神疾病, 2.听力障碍, 3.认知功能障碍, 4.无法配合调查者。

Exclusion criteria：

1.havie serious mental illness; 2.Hearing impairment; 3.Cognitive dysfunction; 4.Unable to co-operate with the survey.
