研究目的:

骨质疏松症和肌肉减少症是老年人跌倒、骨折、独立性丧失及生活质量下降的关键因素。本研究拟开发一种基于CT影像的人工智能预警系统,该系统通过分析常规CT图像,自动测量锥体体积骨密度、椎旁肌肉体积及其脂肪含量,以机会性筛查出潜在的骨质疏松和肌少症高危患者,从而辅助精准诊断,实现早期干预,提升老年人的生活质量。通过整合人工智能技术与现有的医学成像数据,本研究旨在推动一种高效、可持续且精确的医疗保健解决方案,特别针对与年龄相关的肌肉和骨骼退化问题,这不仅有助于提高诊断的准确性,还为老年人群提供了更为全面的健康保障。  

Objectives of Study:

Osteoporosis and sarcopenia are key factors in falls, fractures, loss of independence and reduced quality of life in older adults. This study intends to develop an artificial intelligence early warning system based on CT images. By analyzing conventional CT images, the system automatically measures cone volume bone mineral density, paravertaal muscle volume and fat content, and opportunally screens potential high-risk patients with osteoporosis and sarcosis, thus assisting in accurate diagnosis, achieving early intervention and improving the quality of life of the elderly. By integrating AI technology with existing medical imaging data, this research aims to promote an efficient, sustainable and precise healthcare solution, specifically for age-related muscle and bone degradation, which will not only help improve diagnostic accuracy, but also provide more comprehensive health care for the elderly population.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)2023.01-2024.12在我院接受胸部、腹部CT检查50岁以上的患者,图像需完整包含胸12、腰1椎骨; (2)患者同时机会性做了QCT骨密度检测(拟收集500例); (3)前瞻性招募做了胸部或腹部CT检查的50岁以上志愿者300例,行骨骼肌质量及握力检测,CT与骨骼肌检测间隔时间不超过一个月

Inclusion criteria

(1) Patients over 50 years old who received chest and abdominal CT examination in our hospital from 2023.01 to 2024.12, and the images should completely include thoracic 12 and lumbar 1 vertebrae; (2) QCT bone mineral density test was performed opportunely (500 cases were to be collected); (3) 300 volunteers over 50 years old who underwent chest or abdominal CT examination were prospectively recruited. Skeletal muscle mass and grip strength were detected, and the interval between CT and skeletal muscle detection was not more than one month.

排除标准:

(1)有胸腰椎手术史(包括骨折手术); (2)严重脊柱侧弯; (3)T12、L1、L2的椎体有压缩性骨折或畸形的患者; (4)怀孕或妊娠; (5)图像质量不符合临床要求或影响诊断

Exclusion criteria:

(1) a history of thoracolumbar surgery (including fracture surgery); (2) severe scoliosis; (3) Patients with compression fractures or malformations in the T12, L1, L2 vertebrae; (4) pregnancy or pregnancy; (5) Image quality does not meet clinical requirements or affect diagnosis

金标准或参考标准(即可准确诊断某疾病的单项方法或多项联合方法,在本研究中用于诊断是否有该病的临床参考标准):

1. 骨密度测定 我们以对应的QCT(Mindways, QCTpro, USA)骨密度值作为金标准,其中QCT是药监局审批登记的软件,直接采集QCT骨密度值并对得到的骨密度值进行筛选,去除异常值(小于0的值),最终形成金标准。诊断骨质疏松标准为:骨质正常:>120 mg/cm3;骨量减少:80-120mg/cm3;骨质疏松:<80 mg/cm3。 2. 身高、骨骼肌质量及握力测量 参与者身高数据采用直接询问记录的方式;骨骼肌质量检测采用生物电阻抗(BIA)人体成分分析仪。握力采用握力计。 根据 肌少症诊断标准:同时符合条件(1)和(2):(1)肌肉质量下降:骨骼肌质量指数 SMI(SMI=四肢骨骼肌质量 ASM/身高的平方 H2):低于临界值:男 7.0kg/m2,女 5.7kg/m2;(2)肌肉力量(握力)的降低:男性<26kg,女性<18kg

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

1. Bone density measurement We used the corresponding BMD value of QCT (Mindways, QCTpro, USA) as the gold standard, in which QCT is the software approved and registered by the Food and Drug Administration. We directly collected the BMD value of QCT and screened the obtained BMD value, removed the abnormal value (the value less than 0), and finally formed the gold standard. The criteria for the diagnosis of osteoporosis were: normal bone > 120 mg/cm^3; Bone mass loss: 80-120mg/cm^3; Osteoporosis: < 80 mg/cm^3. 2. Measurement of height, skeletal muscle mass and grip strength The height data of participants were recorded by direct inquiry. Skeletal muscle mass was measured by bioelectrical impedance (BIA) body composition analyzer. Grip strength using a grip dynamometer. According to the diagnostic criteria for sarcopenia, both conditions (1) and (2) were met: (1) decreased muscle mass: skeletal muscle mass index SMI (SMI= ASM of limbs skeletal muscle mass/height square H2) : below the critical value: 7.0kg/m^2 for males and 5.7kg/m^2 for females; (2) The reduction of muscle strength (grip strength) : male < 26kg, female < 18kg.
