研究目的：

探讨飞秒激光辅助小切口角膜基质透镜取出术联合快速角膜交联治疗近视后角膜安全性和有效性。  

Objectives of Study：

To investigate the safety and efficacy of femtosecond laser-assisted small-incision corneal stromal lens removal combined with accelerated corneal cross-linking for the treatment of postmyopic cornea.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

SMILE组： (1)年龄≥18周岁； (2)球镜度度数-1.00D— -10.00D，柱镜度数-5.00D及以下（符合《我国飞秒激光小切口角膜基质透镜取出手术规范专家共识2018年》）； (3)双眼均能良好固视； (4)屈光度稳定2年以上(每年屈光度变化在0.50 D内); (5)角膜：透明无明显云翳或斑翳；角膜地形图检查形态正常，无圆锥角膜倾向; (6)球性软镜停戴2周以上，散光软镜和硬性透氧性角膜接触镜停戴4周以上，角膜塑形镜停戴12周以上。 SMILE Xtra组： (1)除满足以上条件外，还需满足 Corvis生物力学指数(Corvis biomechanical index，CBI)＞0.3，角膜地形图联合生物力学指数(断层扫描生物力学指数)(tomographic biomechanical index，TBI)＞0.3； (2)预期术后角膜基质床厚度>300 μm； (3)角膜最薄点厚度＞400μm。

Inclusion criteria

SMILE group: (1) age >=18 years old; (2) spherical lens degrees -1.00 D- -10.00 D, and columnar lens degrees -5.00 D and below (in accordance with the "Expert Consensus on the Specification of China's Femtosecond Laser Small-Incision Corneal Stromal Lens Removal Surgery 2018"); (3) good fixation in both eyes; (4) refractive stability of more than 2 years (annual refractive change within 0.50 D); (5) Cornea: clear without obvious cloudiness or opacification; normal morphology on corneal topography examination, no tendency to cone cornea; (6) more than 2 weeks of discontinuation of spherical soft lenses, more than 4 weeks of discontinuation of astigmatic soft lenses and rigid oxygen-permeable corneal contact lenses, and more than 12 weeks of discontinuation of keratoplasty lenses. SMILE Xtra group: In addition to the above conditions, (1)Corvis biomechanical index (Corvis biomechanical index (CBI)) > 0.3, corneal topography combined biomechanical index (tomographic biomechanical index (TBI)) > 0.3; (2) expected postoperative corneal stromal bed thickness > 300 μm; (3) corneal thinnest point thickness > 400 μm.

排除标准：

(1)圆锥角膜及亚临床圆锥角膜患者； (2)有眼前节疾病、活动性病变及眼底病变者； (3)有眼部外伤及手术史者； (4)有严重糖尿病、全身结缔组织疾病或自身免疫性疾病者； (5)妊娠及哺乳期妇女； (6)存在焦虑、抑郁等严重心理、精神疾病者。

Exclusion criteria：

(1) patients with keratoconus and subclinical keratoconus; (2) patients with anterior segment disease, active disease and fundus disease; (3) patients with a history of ocular trauma and surgery; (4) patients with severe diabetes, systemic connective tissue diseases or autoimmune diseases; (5) pregnant and lactating women; (6) patients with anxiety, depression and other serious psychological and mental diseases.
