研究目的：

聚焦“十四五”规划中将积极应对人口老龄化上升的国家战略，围绕老年高血压患者的围手术期管理难点，对应项目指南要求，本项目在设计上主要对“老年相关疾病围手术期风险管控关键技术研究”进行了匹配。项目首先建立老年高血压患者可操作性的围手术期评估-干预临床管理方法，形成以MAP为指导的老年高血压患者围手术期血压管理模式，并结合应激变化、炎症变化、术后转归评价，形成老年高血压患者围手术期风险管控评估-干预-评价模式；最后构建适合推广的老年高血压患者围手术期风险关键技术，提高医疗质量的连续性及费-效比，并将研究成果逐步推向基层医疗机构，推动新疆分级诊疗同质化发展,使医疗资源发挥最佳效能。  

Objectives of Study：

Focusing on the national strategy of actively responding to the aging of the population in the 14th Five-Year Plan, focusing on the difficulties in perioperative management of elderly hypertension patients and corresponding to the requirements of the project guidelines, the design of this project mainly matches "research on key technologies of perioperative risk management of elderly-related diseases". The project firstly establishes the operable perioperative risk assessment-intervention clinical management method for elderly hypertension patients, forms the perioperative blood pressure management mode for elderly hypertension patients guided by MAP, and forms the perioperative risk assessment, intervention and evaluation mode of perioperative risk technology for elderly hypertension patients, improve the continuity and cost ratio of medical quality, and gradually push the research results to primary medical institutions, so as to promote the homogenization of hierarchical diagnosis and treatment in Xinjiang, and make the medical resources play the best effectiveness.

纳入标准：

1.年龄≥65岁； 2.在未使用降压药物的情况下，非同日3次测量血压，收缩压 ≥140 和（或）脉压差 ≥60ｍｍ Hg； 3.麻醉方式为全麻的患者； 4.选取 2022年3月-2024年8月于我院外科就诊。

Inclusion criteria

(1) Age>=65 years old; (2) In the absence of antihypertensive drugs, blood pressure was measured 3 times on the same day, with systolic blood pressure >= 140 and/or pulse pressure difference >=60mm Hg; (3) Patients who are anesthetized under general anesthesia; (4)Select the surgical department of our hospital from March 2022 to August 2024.

排除标准：

1.术中发生严重过敏反应患者; 2.患者对试验方法不理解或拒绝试验； 3.有情感障碍等精神疾病史； 4.高危手术患者（如体外循环、主动脉手术、术中发生低血容量休克（HB<90g/L，需输血治疗的患者）、心源性休克、感染性休克、需要循环支持的患者）； 5.MMSE 评分<24，患者因为瞻妄、痴呆及认知障碍不能对并发症及副作用进行回馈； 6.头面部手术不能行脑监测的患者等； 7.其他。

Exclusion criteria：

(1) Patients with severe allergic reactions during surgery; (2) The patient does not understand the test method or refuses the test; (3) Patients with a history of psychiatric disorders such as affective disorders; (4) patients with high-risk surgery (such as cardiopulmonary bypass, aortic surgery, patients with intraoperative hypovolemic shock (HB<90g/L, patients requiring blood transfusion therapy), patients with cardiogenic shock, septic shock, and patients requiring circulatory support) ; (5) MMSE score <24, patients who cannot respond to complications and side effects due to delirium, dementia and cognitive impairment; (6) patients who cannot undergo brain monitoring during cephalofacial surgery; (7) other.
