研究目的：

探讨艾司氯胺酮对单肺通气下胸腔镜手术患者术后肺损伤的影响  

Objectives of Study：

To investigate the effect of esketamine on postoperative lung injury in patients undergoing thoracoscopic surgery under single lung ventilation

药物成份或治疗方案详述：

麻醉诱导给予0.5mg/kg艾司氯胺酮，术中持续静脉泵注0.15mg/kg艾司氯胺酮 

Description for medicine or protocol of treatment in detail：

0.5 mg/kg esketamine was induced by anesthesia, and 0.15 mg/kg esketamine was continuously pumped intravenously 

纳入标准：

（1）年龄30-75岁，男女不限; （2）美国麻醉医师学会（阿萨）身体状况分级系统Ⅱ-III级; （3）身体质量指数（Body mass index,BMI）19~28kg/m^2; （4）择期行胸腔镜下肺癌根治术；

Inclusion criteria

(1) Age 30-75 years old, male or female; (2) United States College of Anesthesiologists (ASA) Physical Status Grading System Level II-III; (3) Body mass index (BMI) 19~28 kg/m^2; (4) Elective thoracoscopic radical lung cancer resection

排除标准：

（1）术前使用放疗、化疗、抗炎药、激素类药物及胸部器官手术史； （2）患有重度COPD、间质性肺疾病、肺大泡或术前肺功能极度减退（FEV1%<40; FVC%<60;） （3）术前有未控制或控制不佳的高血压； （4）合并糖尿病、冠心病、心力衰竭、肝肾功能异常及免疫系统疾病； （5）前有贫血（HB<90g/L）、发热、肺部感染等病史的患者； （6）术中单肺通气（OLV）时气道压> 30 mmHg或SPO2 <90%；

Exclusion criteria：

(1) History of preoperative use of radiotherapy, chemotherapy, anti-inflammatory drugs, hormonal drugs and thoracic organ surgery; (2) Patients with severe COPD, interstitial lung disease, bullae, or extremely low preoperative lung function (FEV1%<40); FVC%<60; ) (3) Uncontrolled or poorly controlled hypertension before surgery; (4) Combined with diabetes, coronary heart disease, heart failure, abnormal liver and kidney function and immune system diseases; (5) Patients with a history of anemia (HB<90g/L), fever, lung infection, etc.; (6) Airway pressure > 30 mmHg or SPO2 <90% during intraoperative single-lung ventilation (OLV);

术前由一位麻醉医生根据分组情况抽取药物，并标记为试验药物，此麻醉医生不参与后续的数据收集。另一位提供麻醉的麻醉医生、患者、 手术医生、麻醉苏醒室护士以及病区护士均对病人分组不知情。数据将由接受过试 验或临床培训的人员收集并进行记录。

Blinding：

Prior to surgery, an anesthesiologist will extract drugs based on grouping and label them as experimental drugs. This anesthesiologist will not participate in subsequent data collection. Another anesthesiologist, patient, surgeon, anesthesia recovery room nurse, and ward nurse who provided anesthesia were unaware of the patient grouping. The data will be collected and recorded by personnel who have received trial or clinical training.
