研究目的：

通过流行病学调查方式，在我国居民中抽取具有代表性的大样本，从宏观的高度和群体的角度去呈现该群体当前的颈椎健康现状，并通过数字化手段建立颈椎健康电子档案信息平台，推动数据互通共享。为实现我国居民的颈椎健康信息有序管理、线上定期随访评估和精准化分类干预奠定基础。  

Objectives of Study：

Through epidemiological investigations, a representative large sample is selected from Chinese residents to present the current cervical health status of this group from a macro and group perspective. A cervical health electronic record information platform is established through digital means to promote data exchange and sharing. To lay the foundation for the orderly management of cervical health information, online regular follow-up evaluation, and precise classification intervention for Chinese residents.

纳入标准：

在调查当地居住时间超过 3 个月（含 3 个月）；年龄≥18 岁，性别不限；自愿参加本研究，并签署知情同意书。

Inclusion criteria

Having resided in the local area for more than 3 months (including 3 months) during the investigation; Age ≥ 18 years old, gender not limited; Voluntarily participate in this study and sign an informed consent form.

排除标准：

数据不完整的调查对象；患有颈部外伤骨折脱位病史；患有严重的颈椎病畸形、肿瘤、结核以及强直性脊柱炎；长期卧床生活不能自理者；患有精神障碍疾病。

Exclusion criteria：

Survey subjects with incomplete data; History of neck trauma, fracture and dislocation; Suffering from severe cervical spondylosis deformities, tumors, tuberculosis, and ankylosing spondylitis; Long term bedridden individuals who are unable to take care of themselves; Suffering from mental disorders.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

所有参加调查的人员均为中医骨伤专业或骨科专业临床医生、研究生。在项目开始前进行统一的培训，通过项目负责人和研究组长的面对面测试后被认为合格。调查前由医生对被调查人群进行颈椎健康相关知识讲解，详细说明本次调查的目的、意义、问卷调查的填写方式；并回答被调查者的疑问，获得社区人群的信任与支持后方可展开调查。由课题组制定《数字化颈椎健康管理指南城镇居民颈椎健康流行病学调查表》（以下简称调查表）。调查表包括人口统计学信息、生活工作习惯信息、颈部功能锻炼信息、颈椎相关症状和体征、颈痛视觉模拟评分、颈椎功能障碍指数评分，SF-36 生活质量评分等内容。调查表由患者在调查医生协助下自行填写，或者由调查医生逐条询问填写。由调查医生对受试者进行如颈肩部压痛点、臂丛牵拉试验等基本体格检查，并如实记录检查结果。对疑似颈椎病者，调查医生结合其既往在医院就诊记录和辅助检查资料进行综合判断，无就诊记录或辅助检查资料者，进一步前往当地医疗机构完善颈椎 X 线检查后，再次判断并记录。同时，在北京市抽样的 4 个社区中，随机抽取 1 个社区进行小样本研究，对该社区中所有被调查者进行颈部肌肉表面肌电、肌氧饱和度检测。所有参与调查的受试者均签署知情同意书。 与相关互联网信息技术人员合作，通过数字化手段，构建颈椎健康电子档案信息平台，并利用数据库的存储功能，实现健康信息资源的传输与共享。平台将被调查者健康信息进行云端化储存和深度智能分析，并为每一位被调查者提供个人账号，其可以登录账号以查看自我健康信息，同时医护人员可以通过平台与被调查者构建联系，实现线上定期随访。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

All participants in the survey are clinical doctors and graduate students majoring in Traditional Chinese Medicine Orthopedics or Orthopedics. Unified training is conducted before the project starts, and it is considered qualified after face-to-face testing by the project leader and research team leader. Before the survey, doctors will provide cervical health-related knowledge explanations to the surveyed population, detailing the purpose, significance, and questionnaire filling method of this survey; And answer the questions of the respondents to gain the trust and support of the community before conducting the investigation. The research group has developed the "Digital Cervical Health Management Guidelines - Epidemiological Survey Form for Cervical Health of Urban Residents" (hereinafter referred to as the survey form). The survey includes demographic information, lifestyle and work habits information, neck function exercise information, cervical related symptoms and signs, neck pain visual simulation score, cervical dysfunction index score, SF-36 quality of life score, and other content. The survey form shall be filled out by the patient with the assistance of the investigating doctor, or by the investigating doctor inquiring and filling out each item. The investigating doctor conducts basic physical examinations on the subjects, such as neck and shoulder tenderness points and brachial plexus traction tests, and truthfully records the examination results. For suspected cervical spondylosis patients, the investigating doctor will make a comprehensive judgment based on their previous medical records and auxiliary examination data. For those without medical records or auxiliary examination data, they will further go to local medical institutions to complete cervical X-ray examination, and then make another judgment and record. At the same time, one community was randomly selected from four sampled communities in Beijing for a small sample study, and all respondents in the community were tested for neck muscle surface electromyography and muscle oxygen saturation. All participants in the survey signed informed consent forms. Cooperate with relevant Internet information technology personnel to build the cervical spine health electronic archives information platform through digital means, and use the storage function of the database to realize the transmission and sharing of health information resources. The platform will store and deeply analyze the health information of the respondents in the cloud, and provide each respondent with a personal account. They can log in to the account to view their own health information. At the same time, medical staff can establish contact with the respondents through the platform to achieve regular online follow-up.
