研究目的：

（1）证实亚麻醉剂量艾司氯胺酮能有效降低老年肌肉减少症患者髋部骨折术后血浆神经酰胺含量，改善术后抑郁焦虑情绪的发生。 （2）揭示神经酰胺及NLRP3炎症因子在术后抑郁焦虑情绪发生中的重要作用 （3）进一步探索术后抑郁焦虑情绪的可能通路与神经酰胺代谢相关，通过神经酰胺介导的NLRP3/Caspase-1/IL-1β通路相关的炎症因子的释放。 （4）证实艾司氯胺酮能通过下调神经酰胺，抑制NLRP3炎症因子的释放，能够降低炎症水平，改善术后抑郁焦虑情绪。  

Objectives of Study：

(1) It was confirmed that the subanesthetic dose of esketamine can effectively reduce the plasma ceramide content and improve the occurrence of postoperative depression and anxiety in elderly patients with sarcopenia after hip fracture. (2) To reveal the important role of ceramides and NLRP3 inflammatory factors in the occurrence of postoperative depression and anxiety (3) To further explore the possible pathway of postoperative depression and anxiety related to ceramide metabolism, through the release of inflammatory factors related to the NLRP3/Caspase-1/IL-1β pathway mediated by ceramide. (4) It was confirmed that esketamine can inhibit the release of NLRP3 inflammatory factor by down-regulating ceramide, reduce inflammation level, and improve postoperative depression and anxiety.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄≥60周岁；ASA分级I~III级；择期行膝关节置换术的老年患者；男性SMI＜42.6cm2/m2女性SMI＜30.6cm2/m2。

Inclusion criteria

Age ≥60 years old; ASA grade I~III; Elderly patients undergoing knee replacement at selected time; Male SMI < 42.6cm2/m2 Female SMI < 30.6cm2/m2.

排除标准：

有恶性高热病史；艾司氯胺酮有使用禁忌证；既往有严重精神疾病的患者，长期服用抗抑郁药物；BMI＞30kg/m2；意识障碍，不能配合完成量表

Exclusion criteria：

A history of malignant hyperthermia; Esketamine has contraindication; Patients with a history of severe mental illness, long-term use of antidepressants; BMI > 30kg/m2; Disturbance of consciousness, inability to complete the scale

Blinding：

The experimental designer used the method of computer generating random numbers to group and number the subjects. The groups were put into sealed envelopes, and the researchers numbered the groups according to the order in which the patients were enrolled and then opened the corresponding envelopes for grouping, without the knowledge of the operators, anesthesiologists, surgeons, postoperative follow-up personnel and data analysts.
