研究目的：

基于ITHBC构建老年慢性心力衰竭运动康复依从性干预方案，并在临床实践中实施方案，通过观察该干预方案对老年慢性心力衰竭患者运动康复依从性、运动康复知信行、运动自我效能感、社会支持及生活质量的影响，评价其应用价值。  

Objectives of Study：

Based on ITHBC, an exercise rehabilitation compliance intervention program for elderly patients with chronic heart failure was constructed, and the implementation plan was implemented in clinical practice, and the effects of the intervention program on exercise rehabilitation compliance, exercise rehabilitation knowledge, belief and practice, exercise self-efficacy, social support and quality of life of elderly patients with chronic heart failure were observed to evaluate its application value.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合《中国心力衰竭诊断和治疗指南2018》中关于CHF的诊断标准； 2.年龄≥60岁； 3.心功能分级I—III级； 4.运动康复依从性量表得分＜44分； 5.有共同居住者（时间＞3个月且能熟练使用电子产品）； 6.同意参加本次研究。

Inclusion criteria

1. Meet the diagnostic criteria for CHF in the Chinese Guidelines for the Diagnosis and Treatment of Heart Failure 2018; 2. Age >= 60 years old; 3. Cardiac function grade I-III; 4. The score of exercise rehabilitation compliance scale < 44 points; 5. Have a co-resident (for > 3 months and be proficient in the use of electronic products); 6. Agree to participate in this study.

排除标准：

1.急性冠脉综合征、新发房颤或房扑、恶性心律失常等运动禁忌症患者； 2.有严重认知障碍者； 3.参加其它康复治疗干预者。

Exclusion criteria：

1. Patients with exercise contraindications such as acute coronary syndrome, new-onset atrial fibrillation or atrial flutter, and malignant arrhythmia ; 2. Those with severe cognitive impairment; 3. Those who participated in other rehabilitation interventions.

为避免研究对象沾染，对同一房间实施相同的干预方法。首先将试点科室的所有病房编号，根据计算机产生随机数字，得出干预组和对照组房间号，对相应的组别实施干预。

To avoid contamination of study subjects, the same intervention was applied to the same room. Firstly, all the ward numbers of the pilot department were numbered, and random numbers were generated according to the computer to obtain the room numbers of the intervention group and the control group, and the corresponding groups were intervened.
