研究目的：

本试验旨在研究在超声可视下引导肺复张操作对妇科腹腔镜手术患者肺不张和氧合的改善情况，探讨超声引导下的肺泡复张手法与标准的肺泡复张手法相比，能否改善妇科腹腔镜手术患者的肺部超声表现并减少围手术期肺不张的发生。  

Objectives of Study：

To investigate the improvement of atelectasis and oxygenation in gynaecological patients undergoing laparoscopic surgeries with ultrasound-guided recruitment manoeuvres, and to investigate whether ultrasound-guided recruitment group manoeuvres can improve lung ultrasound and reduce perioperative atelectasis in patients undergoing gynaecological laparoscopic surgery compared with standard recruitment manoeuvres.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄在18~65岁的患者； 2.手术持续时间至少1小时； 3.美国麻醉医师协会（ASA）分级为Ⅰ、Ⅱ级； 4.无本实验麻醉药品及方式禁忌症。

Inclusion criteria

1. Patients aged 18~65 years; 2. The duration of the operation is at least 1 hour; 3. The American Society of Anesthesiologists (ASA) is graded at Grade I and II; 4. There are no contraindications to anesthetic and methods of this experiment.

排除标准：

1.不签署知情同意书者； 2.有胸部手术史、气胸、既往肺部或心肝肾及神经系统疾病的患者； 3.术前胸片异常或肺功能检查异常者； 4.术前两周内有上呼吸道感染史的患者； 5.肥胖(BMI＜18和（或）＞28kg/m^2)的患者。

Exclusion criteria：

1. Those who do not sign the informed consent file; 2. Patients with a history of thoracic surgery, pneumothorax, previous pulmonary or heart, liver, kidney, and neurological disorders; 3. abnormal chest x-ray or abnormal pulmonary function before surgery; 4. Patients with a history of respiratory tract infections within two weeks before surgery; 5. Patients with obesity (BMI<18(or)>28kg/m^2).

复张时呼吸机设置为压力控制模式（PCV），Pmax设为 40cmH2O，吸气压力设为10~15cmH2O，通气过程中逐步提高PEEP 值，每30s增加5cmH2O，当PEEP值提高到20 cmH2O时，维持此 时的通气状态30s，再每30s降低5cmH2O，直到PEEP水平恢复到复张前的水平。

Intervention：

During reexpansion, the ventilator was set to pressure control mode (PCV), Pmax was set to 40cmH2O, inspiratory pressure was set to 10~15cmH2O, and PEEP value was gradually increased during ventilation, increasing by 5cmH2O every 30 seconds. When PEEP value increased to 20 cmH2O, the ventilation state was maintained for 30 seconds. Another 5cmH2O was reduced every 30 seconds until the PEEP level returned to the pre-reexpansion level.

Intervention：

The reexpansion operation was carried out under the direct real-time guidance of ultrasound. During the reexpansion, the ventilator was set to pressure control mode (PCV), Pmax was set to 40cmH2O, and inspiratory pressure was set to 10~15cmH2O. During the ventilation process, the PEEP value was manually increased by 5cmH2O every 30 seconds, and the ventilation state was maintained for 30 seconds. Until you can't see the collapse on the ultrasound.
