研究目的：

1.证实在下颌阻生第三磨牙拔除手术后，患者拔牙创伤的愈合与去骨、分牙策略选择密切相关。 2.揭示运用刚体力学策略可以缩短下颌阻生第三磨牙拔除手术时间，减少术中、术后并发症的发生率。 3.规范下颌阻生第三磨牙拔除手术中使用刚体力学的方法，形成系统的评价体系，申报牙及牙槽外科新技术，减轻广大患者的拔牙痛苦。  

Objectives of Study：

1. It is confirmed that the healing of tooth extraction trauma after mandibular impacted third molar extraction is closely related to the selection of bone removal and tooth separation strategies; 2. It is concluded that the rigid body mechanics strategy can shorten the operation time of mandibular impacted third molar extraction and reduce the incidence of intraoperative and postoperative complications; 3. Standardize the use of rigid body mechanics in mandibular impacted third molar extraction, form a systematic evaluation system, declare new dental and alveolar surgery techniques, and alleviate the pain of tooth extraction for the majority of patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.患者全身情况良好,IMTMs 及 ASM 局部无炎症,无盲袋; 2.所有拔除的IMTMs 均被粘膜完全阻生,ASM 厚龈生物型; 3.相邻第二磨牙无龋损,无松动,无冠修复体,下颌第一磨牙无缺失; 4.根据 Winter 分类和 Pell 和 Gregory 分级选择 BⅡ近中或水平IMTMs,近中或水平 IMTMs 其颌平面与下颌颌平面的夹角为 11°~100°; 5.IMTM 牙冠近中最凸点至 ASM 远中颈部最凹点的距离 X1-X2(即智齿前间隙)为 1mm; 6.IMTMs 患者自愿参加临床检查和龈下菌斑取样,依从性好,定期随访。

Inclusion criteria

1. The patient was in good general condition with no local inflammation or blind pockets in IMTMs and ASM; 2. All removed IMTMs were completely impacted by mucous membranes, and ASM was a thick gingival biotype; 3. No caries, no loosening, no crown prosthesis, no loss of mandibular first molar; 4. According to the Winter classification and Pell and Gregory classification, select the BⅡ mesial or horizontal IMTMs, and the Angle between the maxillary plane and the mandibular plane of the mesial or horizontal IMTMs is 11°~100°; 5. The distance X1-X2(i.e. front gap of wisdom teeth) from the mesial most convex point of IMTM crown to the most concave point of ASM distal neck is 1mm; 6. IMTMs patients voluntarily participated in clinical examination and subgingival plaque sampling, with good compliance and regular follow-up.

排除标准：

1.患有系统性疾病的患者; 2.ASM 存在病变; 3.依从性差,或未规律治疗者; 4患者自行要求终止试验者或失联无法随访者。

Exclusion criteria：

1. Patients with systemic diseases; 2. There are lesions in ASM; 3. Poor compliance or irregular treatment; 4 Patients who requested termination of the experiment or lost contact could not follow the interview.
