研究目的:

通过多中心的随机对照临床试验,探究无托槽隐形拔牙矫治中基于前牙不同内收方式的更优化的矫治策略,提高隐形拔牙病例的矫治效率。  

Objectives of Study:

A multi-center randomized controlled clinical trial was conducted to explore more optimal treatment strategies based on different anterior teeth retraction methods in tooth extraction cases with clear aligners, so as to improve the treatment efficiency of tooth extraction cases with clear aligners.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)治疗需要拔除上颌或下颌双侧第一前磨牙的错??畸形患者; (2)上颌卵圆或尖圆形弓形,前牙拥挤度≤4mm; (3)前牙覆颌≤5mm; (4)前牙内收量约4-6mm,后牙近中移动量1mm以内; (5)12-45岁,恒牙列,第二磨牙完全萌出; (6)硬腭发育良好,无腭裂,腭皱襞清晰完整; (7)无正畸治疗史,口内牙冠形态良好,无固定修复体; (8)患者自愿接受无托槽隐形拔牙矫治方案; (9)患者依从性良好,可按时复诊。

Inclusion criteria

(1) Treatment of malformed patients who need to have their maxillary or mandibular bilateral first premolar extracted; (2) Maxillary oval or pointed arch shape, anterior teeth crowding degree <= 4mm; (3) Anterior teeth overbite <= 5mm; (4) The amount of anterior tooth adduction is about 4-6mm, and the proximal movement of the posterior teeth is less than 1mm; (5) 12-45 years old, permanent dentition, complete eruption of second molars; (6) The hard palate is well-developed, without cleft palate, and the palatal folds are clear and complete; (7) No history of orthodontic treatment, good dental crown shape in the mouth, and no fixed restorations; (8) The patient voluntarily accepts tooth extraction with clear aligners; (9) The patient has good compliance and can be revisited on time.

排除标准:

(1)患有严重的高血压、糖尿病、冠心病等基础性疾病的患者; (2)服用过影响骨代谢类药物; (3)不能进行X线拍摄的孕妇; (4)存在严重牙周炎和颞下颌关节疾病; (5)各种原因导致的牙冠或牙根形态异常; (6)口内有烤瓷冠、金属冠修复等,进行CBCT拍摄容易形成伪影的患者; (7)患者拒绝接受无托槽隐形拔牙矫治方案; (8)患者依从性差。

Exclusion criteria:

(1) Patients with severe hypertension, diabetes, coronary heart disease and other basic diseases; (2) Have taken drugs that affect bone metabolism; (3) Pregnant women who cannot undergo X-ray imaging; (4) Severe periodontitis and temporomandibular joint disease exist; (5) Abnormal morphology of dental crowns or roots caused by various reasons; (6) Patients with porcelain crowns, metal crowns, and other restorations in the mouth who are prone to developing artifacts during CBCT imaging; (7) The patient refused to accept tooth extraction with clear aligners; (8) Poor patient compliance.

由牵头单位空军军医大学第三附属医院的刘佳宁医生,利用SAS统计分析软件,产生针对未成年人组和成年人组的随机序列,按1:1比例分配给试验组和对照组。

Randomization Procedure (please state who generates the random number sequence and by what method):

Dr. Liu Jianing from the Third Affiliated Hospital of the Air Force Medical University, used SAS statistical analysis software to generate random sequences for the adolescents group and the adult group. It will be assigned to the experimental group and the control group in a 1:1 ratio.
