研究目的：

构建基于应用程序的社区老年人健康照护支持服务的干预方案，并验证其在社区老年人群中应用对老年人生活质量、自我效能感、健康行为的干预效果以及是否能够就降低门诊就诊次数、住院次数。  

Objectives of Study：

To construct an app-based intervention plan for health care support services for older adults in the community, and verify the effect of its application on the quality of life, self-efficacy and health behavior of the elderly in the community, and whether it can reduce the number of outpatient visits and hospitalization.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄>=60岁的社区老年人； 2. 认知、活动功能良好； 3. 处于慢性疾病的稳定期； 4. 拥有并会使用智能手机。

Inclusion criteria

1. Older people aged 60 years or older in the community; 2. Good cognitive and activity function; 3. In the stable phase of chronic disease; 4. Own and use a smartphone

排除标准：

1. 认知障碍（简易精神状态评价量表认知功能障碍评分分数＜27分），或者回答问题时明显存在困难或错误； 2. 身体活动功能受限制（日常生活活动能力量表评分＜75分）； 3. 处于急性期的慢性疾病，如心血管疾病等； 4. 有读写障碍，沟通障碍，无法使用智能手机。

Exclusion criteria：

1. Cognitive impairment (cognitive impairment score < 27 on the Mini-Mental State Examination), or obvious difficulty or error in answering questions; 2. Limited physical activity function ( the Barthel index of ADL < 75); 3. Chronic diseases in the acute phase, such as cardiovascular disease; 4. Dyslexia, communication problems, inability to use smartphones.

该组在社区常规护理服务的基础上，接受本研究开发的社区老年人智能护理程序的干预。采用团体结合个体的形式，拟干预24周，干预频率为每周一次。并且，全科医学科医疗护理团队以及临床营养师将共同通过应用程序对参与者进行远程监测。

On the basis of the community routine nursing service, the group will receive the intervention of the community intelligent nursing program developed in this study. In the form of group and individual, the intervention will be planned for 24 weeks, and the intervention frequency is once a week. 随机方法（请说明由何人用什么方法产生随机序列）：

事先由不参与本研究的课题组其他成员利用Excel2013生成随机数字，进行随机分组。首先对276位研究对象依次编序号01、02、03......276，通过Excel软件中的RAND函数为每位研究对象依次生成对应的随机数。将随机数字由大到小进行排序，按照1:1:1的比例分为3组，每组各92例。

In advance, other members of the research group who did not participate in this study used Excel2013 to generate random numbers for random grouping. Firstly, 276 research objects were numbered 01, 02, 03...... 276. The RAND function in Excel software was used to generate corresponding random numbers for each research object in turn. The random numbers were sorted from largest to smallest and divided into 3 groups according to the ratio of 1:1:1, with 92 cases in each group.
