研究目的：

探究如何优化胸腹联合腔镜食管癌根治术患者术中驱动压导向的肺保护性通气策略，以改善食管癌手术患者的预后。  

Objectives of Study：

To investigate the effect of driving pressure-guided lung protective ventilation strategy on postoperative atelectasis and oxygenation in patients undergoing thoracoscopic combined with laparoscopic esophagectomy, in order to improve the prognosis of patients undergoing esophageal cancer surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄18-75岁；②身体质量指数（BMI）=18.5~25kg/m2；③美国麻醉医师协会分级（American society of anesthesiologists，ASA）Ⅰ~Ⅲ级；④术中单肺通气时间>1h；⑤经医院伦理委员会批准，同意签署书面同意书参与此项研究者

Inclusion criteria

① Age:18-75 years;②BMI=18.5-25kg/m2;③ ASA:I-III;④ time of one lung ventilation>1 hour;⑤ Approved by the hospital ethics committee, the researcher agrees to sign a written consent form to participate in this study.

排除标准：

①合并严重心血管疾病或严重肝肾功能不全患者；②既往严重并发症发生史包括肺部感染、胸腔积液、肺不张、呼吸衰竭、心源性肺水肿；③呼气末正压应用禁忌（张力性气胸，肺大疱，严重气压伤，低血容量性休克，支气管胸膜瘘，高颅内压）；④术中大出血（大于500ml）；⑤血流动力学不稳定

Exclusion criteria：

① Patients with severe cardiovascular disease or severe liver and kidney dysfunction; ② History of serious complications in the past includes pulmonary infection, pleural effusion, atelectasis, respiratory failure, and cardiogenic pulmonary edema Contraindications for the use of positive end expiratory pressure (tension pneumothorax, pulmonary bullae, severe barotrauma, hypovolemic shock, bronchopleural fistula, high intracranial pressure); ④ Intraoperative massive bleeding (greater than 500ml); ⑤ Hemodynamic instability

在胸腔镜和腹腔镜期间均应用最低驱动压力滴定PEEP，具体操作为从0cm H2O开始，每次增加1cmH2O，最高至20cmH2O，每个PEEP水平持续5个呼吸周期，计算各个PEEP值下最后一次呼吸周期时的驱动压值，取最小驱动压时对应的PEEP值为个体化PEEP。

During thoracoscopy and laparoscopy, PEEP titration is applied, starting from 0cm H2O and increasing by 1cmH2O each time, up to a maximum of 20cmH2O. Each PEEP level lasts for 5 respiratory cycles, and the driving pressure value at the last respiratory cycle under each PEEP value is calculated. The PEEP corresponding to the minimum driving pressure is taken as the personalized PEEP.
