研究目的：

（一）总目标 深化老年人失能失智预防干预成果，促进相关知识和技能在社区（村）居民中得到更广泛更有效传播，提高老年人健康素养和健康水平，预防或延缓项目地区老年人失能失智发生，促进实现健康老龄化。 （二）具体目标 1.组织项目培训及地方交流，提升基层医疗卫生服务机构能力； 2.宣传推广并积极推进地方开展失能失智的预防和干预，提高老年人健康素养，预防或延缓失能（失智）的发生； 3.开展随访调查，评估失能失智预防干预措施对认知功能和躯体功能的长期影响。  

Objectives of Study：

(1) Overall goal Deepen the results of disability and dementia prevention interventions for the elderly, promote the wider and more effective dissemination of relevant knowledge and skills among community (village) residents, improve the health literacy and health level of the elderly, prevent or delay the occurrence of disability and dementia among the elderly in the project areas, and promote the realization of healthy aging. (2) Specific objectives 1. Organize project training and local exchanges to improve the capacity of primary medical and health service institutions; 2. Publicize, promote and actively promote the prevention and intervention of disability and dementia in local areas, improve the health literacy of the elderly, and prevent or delay the occurrence of disability (dementia); 3. Conduct follow-up surveys to assess the long-term effects of disability and dementia prevention interventions on cognitive and physical function.

纳入标准：

65-80岁老人； AD8认知障碍自评≥2分或SPPB测试评分在7-11分

Inclusion criteria

65-80 years old; The AD8 self-assessment of cognitive impairment >= 2 or the SPPB test score of 7 to 11

排除标准：

①因躯体或精神原因不能参加干预活动者。 ②正在医疗机构接受躯体/认知功能治疗或康复者

Exclusion criteria：

(1) Those who are unable to participate in the intervention activities due to physical or mental reasons. (2) Those who are receiving somatic/cognitive function treatment or rehabilitation at a medical institution

常规健康教育，在所有干预县（区）进行全人群失能（失智）防控健康教育，内容包括失能失智防控健康教育核心信息、慢性病自我健康管理、健康生活方式、膳食营养、适宜运动等。宣传教育核心材料由中国疾控中心统一提供，鼓励试点地区自行研发具当地特色的公益健康教育材料。

Carry out health education on the prevention and control of disability (dementia) for the whole population in all intervention counties (districts), including the core information of health education on the prevention and control of disability and dementia, self-health management of chronic diseases, healthy lifestyle, dietary nutrition, and appropriate exercise. The core materials for publicity and education are provided by the China Center for Disease Control and Prevention, and pilot areas are encouraged to develop their own public welfare health education materials with local characteristics.

To address the risk factors of disability and dementia, chronic disease management, Eight Section Brocade/Tai Chi exercise, resistance (explosive power) training, relaxation training, weight management, dietary intervention, work and entertainment activities are carried out. Chronic disease management group activities, Eight Section Brocade/Tai Chi exercises, and relaxation training are mandatory activities, while weight management, MIND diet, and work and entertainment activities are optional activities.
