研究目的：

（1）基于心脏磁共振T2*探索地中海贫血等长期输注红细胞的患者心肌铁过载的演进过程，以及铁螯合治疗后的疗效评价。 （2）基于心脏磁共振LVEF，T2*，LGE，T1mapping，ECV 等技术探索地中海贫血等长期输注红细胞的患者心功能及心肌铁过载水平的综合评价策略。 （3）基于心脏磁共振T2*探索输血维持较高基线血红蛋白水平与心肌铁转换率之间的关联。  

Objectives of Study：

(1) To explore the progression of myocardial iron overload in patients with thalassemia and other conditions requiring long-term red blood cell transfusions using cardiac MRI T2*, and to evaluate the therapeutic efficacy after iron chelation treatment. (2) To investigate comprehensive strategies for evaluating cardiac function and levels of myocardial iron overload in patients with thalassemia and other conditions requiring long-term red blood cell transfusions, based on cardiac MRI techniques such as LVEF, T2*, LGE, T1 mapping, and ECV. (3) To explore the association between transfusion-maintained higher baseline hemoglobin levels and the rate of myocardial iron turnover using cardiac MRI T2*.

纳入标准：

1.年龄18-70岁 2.接受定期地长期输血方案

Inclusion criteria

1. Patients aged 18-70 years old; 2. Patients with regular, long-term blood transfusion program.

排除标准：

1. 既往确诊糖尿病，或就诊时达到糖尿病的诊断标准 2. 既往患有心血管疾病、肺部疾病、血栓性疾病、肿瘤等 3. 铁缺乏患者 4. 其他不能配合研究的情况

Exclusion criteria：

1. Patients with previous diagnosis of diabetes, or meet the diagnostic criteria for diabetes at the time of visit 2. Patients with previous cardiovascular diseases, lung diseases, thrombotic diseases, tumors, et al 3. Patients with iron deficiency 4. Patients who are unable to cooperate with the study

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

患者的病史、体格检查、实验室检查、心电图等检查结果采用医院的HIS 系统进行收集，部分病史采用电话随访的形式进行收集；心脏磁共振检查结果采用医院的PACS 系统进行收集，并采用CVI 等后处理工作站进行数据处理。对项目数据收集人员进行培训，使用统一的病例记录表格进行数据收集，并定期审查数据收集的进度和质量，以保证数据收集的标准化、准确性和质量控制。 数据存储采用项目专用的移动硬盘，并定期备份，以防数据丢失。数据采用密码保护，并设置不同研究人员的数据访问权限，必要时进行匿名化处理。数据的保留和使用依照当地的法律和伦理要求。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Patient medical histories, physical examinations, laboratory tests, electrocardiograms, and other examination results are collected using the hospital's HIS (Hospital Information System) system. Some medical histories are collected through telephone follow-ups. Cardiac MRI examination results are collected using the hospital's PACS (Picture Archiving and Communication System) system and processed using post-processing workstations such as CVI. Data collectors are trained to use a unified case record form for data collection, and the progress and quality of data collection are regularly reviewed to ensure standardization, accuracy, and quality control. Data storage is carried out using a project-specific external hard drive, with regular backups to prevent data loss. Data is password-protected, and different data access permissions are set for researchers, with anonymization processed as necessary. The retention and use of data comply with local legal and ethical requirements.
