研究目的：

设置重复性经颅磁刺激（repetitive Transcranial Magnetic Stimulation, rTMS）同部位激活干预，对比常规康复下的单一步行任务训练，观察治疗前后脑卒中患者的下肢运动功能，以验证单双任务步行转换训练疗效与激活靶点的中枢活化机制。  

Objectives of Study：

To set repetitive Transcranial Magnetic Stimulation activation intervention on same brain regions of interest, comparing with pure gait training in regular rehabilitaion. To check clinical efficacy of single-dual task gait trasfer training and central activation mechanism of target points via lower extremities motor function observation of stroke patients before and after treatment.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合脑卒中疾病诊断且经影像学检查确诊；②年龄50周岁以上（考虑脑卒中高发年龄段）；③性别不限；④病程6-24个月；⑤具备步行功能，FIM（Functional Independence Measure, 功能独立性量表）步行项目评分5分及以上；⑥依从性好，愿意完成诊疗；⑦愿意签署知情同意书。

Inclusion criteria

① Match the diagnosis of stroke and confirmed by imaging examination; ② Age above 50 years old (considering the age with high incidence of stroke); ③ Gender is not limited; ④ Course of disease 6-24 months; ⑤ Have walking function, FIM (Functional Independence Measure) walking items score 5 points or above; ⑥ Good compliance, willing to complete diagnosis and treatment; ⑦ Willing to sign informed consent.

排除标准：

①脑卒中原发病情波动进展；②存在严重的、未控制良好的重要脏器疾病，如冠心病、尿毒症等；③严重认知障碍，MMSE（Mini Mental State Exam, 简易精神状态评估量表）评分低于27分（文盲≤17分；小学≤20分；中学（包括中专）≤22；大学（包括大专）≤24分）；④存在严重听觉、言语语言障碍（参见病史或医院相关证明），不能配合完成试验指令。

Exclusion criteria：

① The primary condition of stroke fluctuates and progresses; ② There are serious, uncontrolled important organ diseases, such as coronary heart disease, uremia, etc.; ③ Severe cognitive impairment, MMSE (Mini Mental State Exam) score less than 27 points (illiteracy ≤17 points; Primary school ≤20 points; Secondary schools (including technical secondary schools) ≤22; University (including junior college) ≤24 points); (4) Severe hearing, speech and language disorders (refer to the medical history or relevant hospital certificates), can not cooperate with the completion of the test instructions.

通过Stata 11.0软件分层并生成区组随机数字号（seed=2022），以1:1:1配对进行，课题启动后，按时间顺序每纳入1例患者时，研究人员通过联系专门协调员，按年龄段、同为“脑梗死”、“脑出血”等配对因素以区组的形式入组后进行评定、干预。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The Stata 11.0 software stratified and generated the block random number size (seed=2022), which was performed in 1:1:1 pairing. After the project was started, the researchers contacted a special coordinator to enroll 1 patient in a chronological order. Evaluation and intervention were carried out according to age group and matching factors such as brian infarction and brain hemorrhage.
