研究目的：

本研究旨在探讨心脏手术术后早发型（术后48小时内）和晚发型（术后48小时至7天内）急性肾损伤（AKI）的相关危险因素，并重点分析术中因素的影响，揭示手术过程中可干预的危险因素，帮助临床医生在术中采取有效预防措施。通过建立基于术前和术中数据的机器学习预测模型，并使用SHAP方法解释模型结果，帮助临床医生识别高风险患者，制定个性化的预防和治疗方案，从而改善患者预后。  

Objectives of Study：

The purpose of this study was to investigate the risk factors related to early-onset (within 48 hours after surgery) and late-onset (within 48 hours to 7 days after surgery) of acute kidney injury (AKI) after cardiac surgery, and to focus on the analysis of the influence of intraoperative factors, to reveal the risk factors that can be intervened during surgery, and to help clinicians take effective preventive measures during surgery. By establishing a machine learning prediction model based on preoperative and intraoperative data, and using the SHAP method to interpret the model results, it helps clinicians identify high-risk patients and formulate personalized prevention and treatment plans to improve patient outcomes.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄大于18岁，不限性别，接受心脏外科手术的患者； 2）术前30天内测量过血清肌酐，且在术后48小时内及48小时至7天内至少测量过1次血清肌酐； 3）对于接受多次心脏外科手术的患者，只有第1次手术被纳入分析。

Inclusion criteria

1) Patients over 18 years old, regardless of gender, undergoing cardiac surgery; 2) Serum creatinine has been measured within 30 days before surgery, and serum creatinine has been measured at least once within 48 hours and 48 hours to 7 days after surgery; 3) For patients who underwent multiple cardiac surgeries, only the first surgery was included in the analysis.

排除标准：

1）既往行肾替代治疗、心脏或肾脏移植手术； 2）术前存在终末期肾病或AKI或血清肌酐大于133μmol/L； 3）术前及术后AKI相关指标资料不全； 4）术中血流动力学记录连续10分钟以上不可用； 5）术前需要气管切开术、机械通气或体外膜氧合； 6）接受了自动左心室辅助装置或植入式心律转复除颤器治疗； 7）经历了术中抢救（包括心脏骤停、过敏性休克和大出血）或死亡。

Exclusion criteria：

1) Prior renal replacement therapy, heart or kidney transplantation; 2) Presence of end-stage renal disease or AKI or serum creatinine greater than 133 μmol/L before surgery; 3) incomplete data on preoperative and postoperative AKI-related indicators; 4) Intraoperative hemodynamic records are not available for more than 10 consecutive minutes; 5) preoperative tracheostomy, mechanical ventilation or extracorporeal membrane oxygenation; 6) Received treatment with an automated left ventricular assist device or implantable cardioverter-defibrillator; 7) Experienced intraoperative rescue (including cardiac arrest, anaphylactic shock and hemorrhage) or death.
