研究目的:

1.揭示老年失代偿性心力衰竭肠道粪便微生物结构特点。2.发现差异菌群并分析功能差异,预测通路。3.明确预后相关生物标志物,指导临床。  

Objectives of Study:

1. Reveal the structural characteristics of gut microbiota in elderly patients with decompensated heart failure. 2. Identify differential microbial communities and analyze functional differences to predict pathways. 3. Clarify prognostic biomarkers and guide clinical practice.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄≥60岁的急性失代偿心力衰竭住院患者;2.符合《2021 ESC急慢性心力衰竭指南》中慢性心力衰竭的诊断标准,NYHA分级II至IV级;3.过去近1个月无抗生素与益生菌服用史;4.临床资料完整。

Inclusion criteria

1. Hospitalized patients with acute decompensated heart failure aged ≥ 60 years old; 2. Heart failure was diagnosed according to the 2021 European Society of Cardiology (ESC) guidelines for the diagnosis and treatment of acute and chronic heart failure. All patients were classified within the New York Heart Association(NYHA) functional classes II to IV; 3. No history of taking antibiotics or probiotics in the past month; 4. Complete clinical data.

排除标准:

1.急性冠脉综合征;2.合并肝硬化(Child‐Pugh评分 B 或C)、严重脓毒症、恶性肿瘤、慢性肾脏病5期或严重慢性消耗性疾病;3.标本收集前1个月应用抗生素或益生菌的;4.依从性差,难以随访的患者。

Exclusion criteria:

1.Acute coronary syndrome; 2.Severe liver cirrhosis (Child–Pugh score B or C), severe sepsis, malignant tumours or stage 5 chronic kidney disease; 3.Use antibiotics or probiotics one month before specimen collection; 4.loss of follow-up.

金标准或参考标准(即可准确诊断某疾病的单项方法或多项联合方法,在本研究中用于诊断是否有该病的临床参考标准):

心力衰竭诊断符合《2021 ESC急慢性心力衰竭诊断和治疗指南》中的标准。患者具备心衰的症状及体征,并且满足肺淤血或心功能不全客观证据:如 NT-ProBNP 升高等征象。所有患者均使用静脉注射利尿剂治疗。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The diagnosis of HF was based on the 2021 European Society of Cardiology Guidelines. HF was defined as the presence of symptoms or signs associated with HF and evidence of cardiac dysfunction. Objective evidence of pulmonary congestion or cardiac dysfunction in the patient, including the increased plasma concentrations of N-terminal pro-B-type natriuretic peptide (NT-proBNP). All patients were treated with intravenous diuretics.

采用16S rRNA二代高通量测序技术明确肠道菌群结构特点。提取粪便样本的基因组,使用DNA Nanodrop 2000检测DNA质量和浓度。使用引物338F和806R扩增细菌16S核糖体RNA基因的V3-V4区。PCR产物使用NEB Next Ultra II DNA Library Prep Kit进行文库构建。然后在Illumina Miseq/Novaseq 6000平台测序。完成测序后,对所得数据进行生物信息学分析,使用PICRUSt2软件对粪便基因组数据进行菌群代谢功能预测。

Index test:

Using 16S rRNA second-generation high-throughput sequencing technology to clarify the structural characteristics of gut microbiota. Extract the genome of fecal samples and use DNA Nanodrop 2000 to detect DNA quality and concentration. Use primers 338F and 806R to amplify the V3-V4 regions of bacterial 16S ribosomal RNA genes. PCR products were constructed using the NEB Next Ultra II DNA Library Prep Kit for library construction. Then sequence on Illumina Miseq/Novaseq 6000 platform. After completing the sequencing, bioinformatics analysis was performed on the obtained data, and PICRUSt2 software was used to predict the microbial metabolism function of the fecal genome data.
