研究目的:

精神分裂症(SCZ)以思维、情感、行为、认知等方面的异常为主要临床特点,发病机制不明。使用啮齿类动物作为SCZ的疾病模型存在脑环路基础的天然局限,无法准确模拟人类SCZ的病理过程。寻求新型动物品系、建立模型是解决当前困扰的发展趋势,创建云南特色准“灵长类”动物树鼩的新SCZ模型,用以研究精神障碍发病机制优势显著。申请人前期参照触摸屏系统,根据树鼩的天然特性设计建立了符合其生物特点的认知评价体系,包括树鼩特色专属认知迷宫、夜间睡眠及出巢频率评价、进食量评价,重复性、灵敏度均较理想。本研究拟创建药物诱导和脑区活动性调控两种树鼩SCZ模型,并进一步完善行为学评价体系,综合利用PET-CT、病毒示踪、基于化学遗传学的脑区活性调控等手段探索SCZ的神经环路机制。在此基础上,探索利用PET-CT和分子生物学方法研究SCZ非药物治疗手段的制定策略,以期能为临床更深入的理解SCZ的发病机制尽一份绵薄之力! 基于这一研究项目,我们的研究主要从临床研究和动物实验逐步进行探索。精神分裂症患者体内与能量产生相关的基因选择性下调,这与锥体神经元发射的长期减少相一致,而调节脑能量代谢可能是治疗精神分裂症的有效方法。此外,研究还表明,精神分裂症的病理机制与能量代谢障碍、线粒体功能障碍、神经炎症、氧化还原失调和 NMDAR 功能低下等多种因素有关,因此大脑在神经发育过程中构成了一个以氧化应激为中心的恶性循环,这些病理过程在大脑中形成正反馈循环,相互影响、相互放大,这是精神分裂症特征性感觉、认知、情感和社交障碍的基础。乳酸是生物能量供应的重要组成部分,其代谢紊乱与多种精神疾病有关。首先我们探索的是精神分裂症患者乳酸代谢是否异常,在树鼩新型精神分裂症模型的建立取得突破性成果后,检测该模型的乳酸代谢是否异常。  

Objectives of Study:

Schizophrenia (SCZ) is characterized by abnormalities in thinking, emotion, behavior, and cognition as the main clinical features, and the pathogenesis is unknown. The use of rodents as a disease model of SCZ has the natural limitation of brain loop basis and cannot accurately simulate the pathological process of human SCZ. The search for new animal strains and the establishment of models is the development trend to solve the current problems, and the creation of a new SCZ model of Yunnan characteristic quasi-"primate" animal tree shrew, which can be used to study the pathogenesis of mental disorders, has significant advantages. The applicant has designed and established a cognitive evaluation system based on the natural characteristics of the tree shrew, including the unique cognitive maze, the evaluation of nighttime sleep and nesting frequency, and the evaluation of food intake, which is characterized by the tree shrew's unique cognitive maze, and the repeatability and sensitivity are satisfactory. In this study, we intend to create two kinds of SCZ models for tree shrews, drug-induced and brain region activity regulation, and further improve the behavioral evaluation system to explore the neural loop mechanism of SCZ by comprehensively utilizing PET-CT, viral tracing, and chemical genetics-based brain region activity regulation. On this basis, we will explore the use of PET-CT and molecular biology methods to study the development of strategies for non-pharmacological treatments for SCZ, with a view to making a modest contribution to a deeper clinical understanding of the pathogenesis of SCZ! Based on this research program, our study is mainly explored from clinical studies and animal experiments step by step. Selective down-regulation of genes associated with energy production in schizophrenia patients is consistent with a long-term reduction in pyramidal neuron firing, and modulation of brain energy metabolism may be an effective treatment for schizophrenia. In addition, studies have shown that the pathological mechanisms of schizophrenia are related to a variety of factors such as impaired energy metabolism, mitochondrial dysfunction, neuroinflammation, redox dysregulation, and NMDAR hypofunction, and thus the brain constitutes a vicious cycle centered on oxidative stress during neurodevelopment, whereby these pathological processes form positive feedback loops in the brain, interacting with each other and amplifying each other, which is the main cause of schizophrenia's basis for the characteristic sensory, cognitive, emotional, and social deficits. Lactate is an important component of the bioenergy supply, and its metabolic disorders are associated with a variety of psychiatric disorders. We first explored whether lactate metabolism is abnormal in patients with schizophrenia, and tested whether lactate metabolism is abnormal in a novel schizophrenia model of the tree shrew after a breakthrough in the establishment of this model.

纳入标准:

精神分裂症患者纳入标准:患者的诊断严格遵循美国精神障碍诊断统计手册第五版(Diagnosticand statistical manual of mental disorders, Fourth edition, DSM-V)中关于精神分裂症的诊断标准;年龄18-60岁;性别不限;民族均选择汉族。

Inclusion criteria

Inclusion criteria for patients with schizophrenia: the diagnosis of patients strictly followed the diagnostic criteria for schizophrenia in the Diagnostic and statistical manual of mental disorders, Fourth edition (DSM-V) of the United States of America; age 18-60 years old; sex No limitation; Han nationality was chosen for all ethnic groups.

排除标准:

精神分裂症患者排除标准:无精神发育迟滞或其它精神疾病史;一周前无感染或酒精和/或精神活性物质滥用史;无病毒性脑炎、糖尿病、甲状腺功能异常、高血压、脂肪肝、痛风、心功能衰竭、肾功能衰竭或其他内科疾病;排除坐轮椅/卧床/有进食障碍的患者、妊娠期/哺乳期的女性患者以及曾经/正在使用降血糖、血脂或血压相关药物的患者。

Exclusion criteria:

Exclusion criteria for patients with schizophrenia: no history of mental retardation or other psychiatric disorders; no history of infection or abuse of alcohol and/or psychoactive substances prior to one week; no viral encephalitis, diabetes mellitus, thyroid dysfunction, hypertension, fatty liver, gout, cardiac failure, renal failure, or other medical illnesses; exclusion of wheelchair-bound/bedridden/feeding-disordered patients, pregnant/lactating female patients and patients who have used/are using medications related to lowering blood glucose, blood lipids or blood pressure.

A master's degree student accessed the medical record system of the Yunnan Provincial Psychiatric Hospital and recorded the basic information and biochemical test results of the schizophrenia patients to be studied. To ensure the accuracy and consistency of the diagnosis, we then had another master's degree student with extensive clinical experience review, double-check, and enter the patient's medical record information in detail.
