研究目的：

評估以 WHO ICOPE 框架為基礎的社區多元干預措施對改善衰弱老年人健康（身體、認知、營養、心理、社交網絡）結果的有效性。  

Objectives of Study：

To assess the effectiveness of a community-based multicomponent intervention based on the WHO ICOPE framework on improving health outcomes among older adults with frailty.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

需同時滿足以下所有條件： 1. 年齡65歲以上； 2. 滿足一或兩項衰弱表型評分（即衰弱前期、衰弱）； 3. 認知能力下降（MoCA > 10）； 4. 無需他人協助即可自行行走； 5. 居住於澳門； 6. 具備粵語或國語溝通能力； 7. 願意在知情同意的情況下參與干預計畫。

Inclusion criteria

All the following conditions must be met at the same time: 1. Aged 65 years and above; 2. Meet one or two frailty phenotype score (i.e. pre-frail, frail); 3. Cognitive decline (MoCA > 10); 4. Able to ambulate without personal assistance; 5. Residing in Macao; 6. Ability to communicate with Cantonese or Mandarin; 7. Willing to participate in the intervention program with informed consent.

排除标准：

任一以下條件： 1. 自我報告的嚴重醫療狀況和精神疾病（與心臟病相關的嚴重疾病、慢性阻塞性肺病（COPD）、中風、癌症、氣喘、嚴重腎功能障礙、末期疾病、重度憂鬱症、躁鬱症、強迫症與創傷後障礙）； 2. 臥床； 3. 認知障礙（MoCA≤10）； 4. 感覺障礙（視覺和聽覺）妨礙溝通； 5. 已參與或仍在參與其他健康計畫和研究； 6. 任何持續性骨折； 7. 過去六個月內曾接受過任何手術（如臗、腹部）。

Exclusion criteria：

Any of the following conditions: 1. Self-reported severe medical conditions and mental illnesses (heart-related severe disease, chronic obstructive pulmonary disease (COPD), stroke, cancer, asthma, renal severe dysfunction, terminally ill, major depression, bipolar, disorder, obsessive compulsive disorder and post-traumatic disorder); 2. Bed-ridden; 3. Cognitive impairment (MoCA ≤ 10); 4. Sensory impairment (visual & hearing) that interferes communication; 5. Already involved or still participating in other health program and study; 6. Any sustained fracture; 7. Any surgery (e.g. hip, abdominal area) in past six months.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

數據採集：本研究之數據收集將採取兩種模式（電子和紙本）：大部份問卷將使用「問卷星」（https://www.wjx.cn/）收集，某些評估表牽涉畫圖，如MoCA，則會使用紙本作數據收集。紙本數據將錄入到電腦中儲存並作後續分析。 管理：本研究會給予每位參與者一個編號，使其之個人資料無法透過收集之資料被識別。而參與者之數據（包括電子和紙本）的管理將根據澳門個人資料保護法管理，並按澳門鏡湖護理學院對資料管理的管理辦法，在妥善保存五年後銷毁。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data Collection: The data collection for this study will adopt two modes (electronic and paper-based): Most of the questionnaires will be collected using "W" (https://www.wjx.cn/), while some assessment forms involving drawing, such as MoCA, will use paper-based data collection. The paper-based data will be entered into the computer for storage and subsequent analysis. Management: This study will assign a number to each participant, so that their personal information cannot be identified through the collected data. The participant data (including electronic and paper-based) will be managed in accordance with the "Macao Personal Data Protection Act", and will be properly preserved for five years and then destroyed, following the data management regulations of the Kiang Wu Nursing College of Macau.
