研究目的：

针对具有淋巴结转移风险的早期胃癌患者，通过探索内镜联合腹腔镜手术治疗的方式，建立消化道早癌内外科联合治疗的新术式。以5年复发率为标准，验证新术式的安全性及有效性，进一步细化早期胃癌治疗适应证，实现早期胃癌的精准保功能治疗。  

Objectives of Study：

Aiming at the early gastric cancer patients with the risk of lymph node metastasis, a new operation method of endoscope combined with laparoscopic surgery was established for the combined treatment of early gastrointestinal cancer. The 5-year recurrence rate was used as the standard to verify the safety and effectiveness of the new operation, further refine the indications for the treatment of early gastric cancer, and achieve accurate functional preservation therapy for early gastric cancer.

纳入标准：

①年龄：18 岁-80 岁（包含 18、80岁）；②性别不限；②早期胃癌ESD相对扩大适应证或ESD非治愈性切除术后；③内镜下抬举征阳性；④体力评分：ECOG≤2 分，可耐受标准胃癌根治术；⑤无胃肠道手术史，无化疗或放疗史；⑥肝、肾、心、肺、骨髓功能正常（GPT4×109/L，PLT＞100×109/L）；⑦能够理解并遵守研究方案，同意并签署知情同意书。

Inclusion criteria

① Age: 18-80 years old (including 18 and 80 years old); ② Gender is not limited; ② Early gastric cancer after ESD relative enlargement indication or ESD non-curable resection; ③ endoscopic lift sign is positive; ④ Physical strength score: ECOG≤2 points, can tolerate standard radical gastrectomy; ⑤ No history of gastrointestinal surgery, chemotherapy or radiotherapy; Liver, kidney, heart, lung, bone marrow function was normal (GPT4×109/L, PLT > 100×109/L); Able to understand and abide by the research protocol, agree and sign the informed consent.

排除标准：

①无法确定示踪剂染色范围的患者；②示踪剂使用禁忌或胃镜检查禁忌；③未能接受ESD治疗术或ESD术失败；④凝血功能障碍、心肺功能不全等外科手术禁忌者或有未控制的癫痫、中枢神经系统疾病或精神障碍史者或器官移植需要免疫抑制治疗者；⑤妊娠期、哺乳期，有受孕可能未采取有效避孕措施者及准备受孕者；⑥同时患有其他系统恶性肿瘤者；或术中证实为转移性胃癌或非胃癌；⑦同时参加其他临床试验。

Exclusion criteria：

① Patients whose tracer staining range cannot be determined; ② The use of tracers or gastroscopy contraindications; (3) Failure to receive ESD treatment or ESD failure; (4) Coagulopathy, cardiopulmonary insufficiency and other surgical contraindication, or uncontrolled epilepsy, central nervous system disease or mental disorder history, or organ transplantation requires immunosuppressive treatment; (5) Pregnancy, lactation, pregnancy may not take effective contraceptive measures and prepare to conceive; ⑥ Patients with other systemic malignancies; Or the intraoperative evidence was metastatic gastric cancer or non-gastric cancer; ⑦ Participate in other clinical trials.

随机分组：为保证受试者组间临床资料的匹配，本研究采用区组随机法将受试者随机分配至试验组和对照组，随机比例为1:2。试验组采用内镜-腹腔镜胃区域性切除联合前哨淋巴结清扫术；对照组采用标准D2胃癌根治术。本中心人员记录并填写如下信息后进行分组。

Randomization: In order to ensure the matching of clinical data among the subjects, this study adopted block randomization method to randomly assign the subjects to the test group and the control group, and the randomization ratio was 1:2. The experimental group underwent endoscopic laparoscopic regional gastrectomy combined with sentinel lymph node dissection. The control group underwent standard D2 radical gastrectomy. The center personnel record and fill in the following information before grouping.

盲法：

目前的研究是单盲试验；因此，患者将不知道他们的分组。外科医生会根据患者分组进行相应的手术治疗。患者出院后，将手术方法以病历的形式发给患者。

Blinding：

The present study is a single-blind trial; therefore, the patients will not know their grouping. The surgeon will carry out the corresponding surgical treatments according to the patients’ grouping. After patients are discharged from the hospital, the surgical methods will be sent to them in the form of medical records.
