Objectives of Study：

Evaluation of the recommended phase 2 dose of KN046 in combination with chemotherapy and radiation therapy; Evaluate the antitumor activity of KN046 in combination with chemotherapy and radiotherapy;

Description for medicine or protocol of treatment in detail：

 

纳入标准：

I01. 受试者能够理解知情同意书，自愿参与并签署知情同意书；
I02. 受试者签署知情同意书当天≥18周岁且≤75周岁，男女不限；
I03. 组织学确认的复发或转移性食管鳞癌；
I04. 根据RECIST 1.1标准，在基线至少有1个可测量病灶；
I05. ECOG评分0或1分；
I06. 足够的肝、肾、骨髓功能

Inclusion criteria

1. Subjects can understand the informed consent, volunteer to participate and sign the informed consent;
2. On the day the subject signs the informed consent, aged 18 to 75 years;
3. Histologically confirmed recurrent or metastatic esophageal squamous cell carcinoma;
4. According to the RECIST 1.1 standard, there is at least 1 measurable lesion at baseline;
5. ECOG 0 or 1;
6. Adequate function of liver, kidney and bone marrow.

排除标准：

E01. 未经治疗的活动性脑转移受试者或脑膜转移受试者；
E02. 给药前28天内参加过任何其他干预性临床试验；
E03. 给药前28天内接受过其他抗肿瘤治疗；
E04. 给药前28内接受过重大手术治疗；
E05. 给药前3个月内接受过根治性放疗；
E06. 既往接受过免疫检查点阻断剂或T细胞共刺激药物治疗等；
E07. 给药前14天内需要接受系统性皮质激素；
E08. 给药前28天内接受过活疫苗（包括减毒活疫苗）接种；
E09. 既往或目前患有间质性肺炎/肺病；
E10. 既往或目前患有自身免疫性疾病；
E11. 首次给药前5年内合并其他恶性肿瘤；
E12. 患有未经控制的合并症，包括但不限于以下情况：
? 活动性HBV或HCV感染；
? 已知HIV感染或AIDS病史；
? 活动性结核病；
? 活动性感染；
? 未能控制的高血压；
? 严重性心脏疾病；
? 活动性出血；
E13. 既往抗肿瘤治疗的毒性未恢复至CTCAE≤1级（NCI-CTCAE v5.0）；
E14. 既往异基因骨髓或器官移植病史；
E15. 既往对抗体类药物过敏反应、不耐受病史；
E16. 怀孕和/或哺乳期女性；
E17. 有瘘管病史或可能发生瘘；
E18. 在随机之前重度营养不良；
E19. 其他研究者认为会影响本研究药物治疗安全性或依从性的情况。

Exclusion criteria：

E01. Untreated subjects with active brain metastases or meningeal metastases;
E02. Participated in any other interventional clinical trials within 28 days before the study;
E03. Received other anti-tumor treatments within 28 days before the study;
E04. Underwent major surgical treatment within 28 days before the study;
E05. Received radical radiation therapy within 3 months before the study;
E06. Previously received immune checkpoint blockers or T-cell co-stimulation drugs;
E07. Received systemic corticosteroids within 14 days before the study;
E08. Received live vaccines (including live attenuated vaccines) within 28 days before the study;
E09. Past or present history of interstitial pneumonia / pulmonary disease;
E10. Past or present history of autoimmune disease;
E11. Combined with other malignant tumors within 5 years before the first administration;
E12. Suffering from uncontrolled comorbidities, including but not limited to the following:
? active HBV or HCV infection;
? Known history of HIV infection or AIDS;
? active tuberculosis;
? active infections;
? uncontrolled hypertension;
? severe heart disease;
? active bleeding;
E13. The toxicity of previous antitumor treatment has not returned to CTCAE ≤Gr 1;
E14. History of previous allogeneic bone marrow or organ transplant;
E15. A history of allergic reactions and intolerance to antibody drugs;
E16. Pregnant and / or breastfeeding women;
E17. History of fistula or prone to fistula;
E18. Severe malnutrition before randomization;
E19. Conditions that may affect the safety or adherence in this study.
