1）构建基于PERMA模型的老年脑卒中患者失志综合征的干预方案构建。 2）探讨基于PERMA模型的老年脑卒中患者失志综合征干预方案的应用效果。  

Objectives of Study：

1) To construct the construction of an intervention program based on the PERMA model for the lost-will syndrome in elderly stroke patients. (2) To explore the application effect of the intervention program of lost consciousness syndrome in elderly stroke patients based on PERMA model.

药物成份或治疗方案详述：

纳入标准：

（1）年龄≥60周岁； （2）意识清醒并且生命体征平稳，能理解及配合检查； （3）符合《中国急性缺血性脑卒中诊治指南2018》中的诊断标准[139]； （4）经脑CT或脑MRI检查，明确有出血性或缺血性病灶； （5）自愿签署知情同意书的患者。

Inclusion criteria

(1) Age ≥ 60 years; (2) Conscious and stable vital signs, able to understand and cooperate with the examination; (3) Meet the diagnostic criteria in the Chinese Guidelines for Diagnosis and Treatment of Acute Ischemic Stroke 2018 [139]; (4) Clearly have hemorrhagic or ischemic lesions by brain CT or brain MRI; (5) Patients who voluntarily signed an informed consent form.

排除标准：

（1）既往有重度抑郁症或其他精神疾病； （2）伴有严重心、肝、肾等重要脏器功能不全，既往有器质性脑血管疾病； （3）聋哑、理解障碍、意识不清可能影响填写调查问卷及访问的患者； （4）接受过心理干预或者正在进行心理干预的患者。

Exclusion criteria：

(1) Prior major depression or other psychiatric disorders; (2) With severe cardiac, hepatic, renal and other vital organ insufficiency and previous organic cerebrovascular disease; (3) Patients with deafness, comprehension disorders, or unconsciousness that may affect the completion of questionnaires and interviews; (4) Patients who have received psychological intervention or are undergoing psychological intervention. 对照组给予患者神经内科常规护理，具体内容如下： （1）一般护理：密切监测患者的生命体征，包括意识、瞳孔、呼吸、血压、心率等，及时发现并处理异常情况。保持呼吸道通畅，预防并发症。 （2）用药护理：按照医嘱准确给予药物和输液，确保用药安全和有效。对于有过敏、中毒反应等情况，应立即停药和处理。 （3）康复治疗：根基患者具体情况制定具体康复治疗方案，包括早期康复训练、针灸治疗、中药外敷、肢体功能训练、吞咽功能训练等。 （4）饮食护理：保持低盐、低脂、高蛋白以及高维生素饮食，避免高盐、高胆固醇、高脂肪的食物。 （5）生活护理：勤换衣物，清洁皮肤，以免引起压疮；防止患者跌倒坠床风险；纠正患者不良生活习惯，如戒烟、限酒、控制体重等。 （6）心理护理：密切关注患者的心态变化，注重人文关怀，做好心理疏导工作。鼓励患者建立信心，尊重患者，耐心倾听患者诉说，鼓励患者表达自己的感受，积极面对疾病。 （7）健康宣教：向患者和家属宣传疾病知识和健康生活方式，提高患者自我保健意识。
