本研究将基于循证医学，构建以护士为主导的ICU机械通气成人患者的撤机方案，并将方案进行临床实证研究。  

Objectives of Study：

Based on evidence-based medicine, this study will build a nurse-led ICU mechanical ventilation withdrawal plan for adult patients, and conduct clinical empirical research on the plan.

纳入标准：

①经口鼻气管插管应用机械通气的成年患者（≥18岁）； ②机械通气时间＞24h； ③知情、签署同意书者。

Inclusion criteria

1. Adult patients with mechanical ventilation through oral and nasal tracheal intubation (≥ 18 years old); 2. Mechanical ventilation time > 24h and ≤7d; 3. Those who know and sign the consent form.

排除标准：

①研究期间死亡； ②研究期间行气管切开者； ③研究期间非计划出院者。

Exclusion criteria：

1. Death during the study period.; 2. Internal tracheotomy during the study period; 3. Unplanned dischargers during the study period.

采用以护士为主导的ICU成人机械通气患者撤机方案。撤机循证方案构建完成后，在实施前，对科室医护人员进行相关知识教育培训。重点对医护人员进行机械通气撤机的相关知识培训，将方案中使用的工具量表发给科室医护人员并组织统一学习，其次对方案内容进行具体讲解，使符合条件的医护人员熟悉掌握相关知识，并进行考核。经过教育培训后，于徐州医科大学附属医院重症医学科实施方案，方案小组成员提供指导和监督，并做好采集资料数据的工作。

Intervention：

Adopt the nurse-led ICU adult mechanical ventilation patient withdrawal plan. After the construction of the withdrawal evidence-based program is completed, relevant knowledge education and training will be carried out for the medical staff of the department before implementation. Focus on the relevant knowledge training of mechanical ventilation and withdrawal for medical staff, send the tool scale used in the program to the medical staff of the department and organize unified learning, and then explain the content of the plan in detail, so that qualified medical staff can be familiar with the relevant knowledge and conduct an assessment. After education and training, the program members of the program team provided guidance and supervision in the Department of Critical Care Medicine of the Affiliated Hospital of Xuzhou Medical University, and did a good job in collecting data.
