研究目的：

本研究旨在分析CRE所致感染的病原学特点、优势种群、危险因素、临床特点、诊疗方案及预后。通过深入研究这些方面，为CRE所致感染的防控及诊治提供参考依据，包括下列主要目的： （1） 建立双向转诊背景下预防和控制CRE的联合网络。 （2） 调查双向转诊背景下CRE的环境流行病学和传播动态。 （3） 最大限度地发挥多学科团队在常规CRE预防和控制中的作用。 本项目期望能获得双向转诊情境中CRE所致感染的全面和系统的科学数据，形成双向转诊中CRE预防和控制的专家共识，并能进一步影响医疗管理和行政政策制定。  

Objectives of Study：

The aim of this study is to analyze the pathogenic characteristics, dominant populations, risk factors, clinical features, diagnosis and treatment plans, and prognosis of CRE induced infections. By conducting in-depth research on these aspects, we aim to provide reference for the prevention and treatment of CRE induced infections, including the following main objectives: (1) Establish a joint network for preventing and controlling CRE in the context of two-way referral. (2) Investigate the environmental epidemiology and transmission dynamics of CRE in the context of two-way referral. (3) Maximize the role of multidisciplinary teams in routine CRE prevention and control. This project aims to obtain comprehensive and systematic scientific data on CRE induced infections in the context of two-way referral, form expert consensus on CRE prevention and control in two-way referral, and further influence medical management and administrative policy formulation.

药物成份或治疗方案详述：

（1）第一部分：回顾性研究 建立双向转诊背景下CRE预防和控制的联合网络，并对联合网络中CRE传输的负担进行横断面调查。计划回顾性收集2023.01-2024.06期间通过双向转诊在四川大学华西医院及其转诊医院就诊的携带CRE的患者资料及双向转诊来源、途径、菌种类型等信息，共计纳入约1000名。 （2）第二部分：前瞻性研究 计划从2026年1月至2027年06月期间，计划在四川大学华西医院及其转诊医院招募双向转诊的携带CRE的患者共约1000名。

纳入标准：

①临床CRE所致感染，有相应临床症状或影像学支持； ②至少一次合格标本培养或NGS提示相应的病原学结果； ③通过双向转诊来院就诊；

Inclusion criteria

① Infection caused by clinical CRE, with corresponding clinical symptoms or imaging support; ② At least one qualified specimen culture or NGS prompt corresponding pathogen results; ③ Visit the hospital through two-way referral;

排除标准：

回顾性研究部分：临床资料缺失明显或资料可信度较低的病例； 前瞻性研究部分：患者或家属无民事行为能力，无法进行知情同意。

Exclusion criteria：

Retrospective study section: Cases with significant missing clinical data or low data reliability; Prospective study section: Patients or their families lack civil capacity and are unable to provide informed consent.
