研究目的：

本研究旨在探讨真实世界中急性后循环大梗死核心卒中患者接受血管内治疗的安全性和有效性, 为这部分临床尚缺乏有效疗法的患者提供循证医学证据，同时为后期进一步开展多中心随机对照研究提供理论支持。  

Objectives of Study：

The aim of this study is to explore the safety and effectiveness of endovascular treatment for acute posterior circulation stroke patients with large infarct core in the real world, providing evidence-based medicine for those patients who still lack effective treatment in clinical practice.This will provide theoretical support for further multicenter randomized controlled studies in the future.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥18周岁； 2. 到院时间距离最后正常时间在24小时以内； 3. DSA/CTA/MRA检查证实后循环大血管闭塞； 4. CT/MRI下后循环ASPSECTS评分0-5分； 5. 美国国立卫生研究院卒中量表（NIHSS）评分 ≥ 6分； 6. 患者或家属签署书面知情同意书。

Inclusion criteria

1. Ages ≥ 18 years; 2. Present less than 24 hours of stroke onset or last seen well; 3. Clinical diagnosis of AIS due to posterior circulation large vessel occlusion on DSA/CTA/MRA; 4. Alberta Stroke Program Early CT Score (ASPECTS)≤5 on CT/MRI; 5. National Institute of Health stroke scale (NIHSS) ≥6; 6. Informed consent obtained from patients or their legal representatives.

排除标准：

1. 经头颅计算机断层扫描（CT）或核磁共振成像（MRI）证实颅内出血； 2. 接受静脉溶栓治疗; 3. 发病前mRS评分≥2分； 4. 妊娠或哺乳期妇女； 5. 造影剂过敏； 6. 正在参加其他临床试验； 7. 收缩压>185 mmHg，或舒张压>110 mmHg，且口服降压药物无法控制 8. 遗传学或获得性出血体质，抗凝因子缺乏；或已使用抗凝药且INR>3； 9. 血糖<50mg/dl（2.8mmol/L）或>400mg/dl（22.2mmol/L）,血小板<100*109/L，或者红细胞压积<25%； 10. 近1个月有出血史； 11. 任何疾病晚期致预期寿命<6个月； 12. 预期不能完成随访； 13. 颅内动脉瘤、动静脉畸形； 14. 影像学上具有占位效应的颅内肿瘤（微小脑膜瘤除外）； 15. 脑疝导致颅脑中线移位； 16. 含有严重并发症可能影响患者预后和随访（癌症、严重心衰、肾衰）。

Exclusion criteria：

1. Confirmed intracranial hemorrhage through computed tomography (CT) or magnetic resonance imaging (MRI); 2. Received Intravenous Thrombolysis; 3. Pre-stroke modified Rankin Scale (mRS) score ≥ 2; 4. Pregnancy or breastfeeding women; 5. Allergy to contrast agents; 6. Currently participating in other clinical trials; 7. Systolic blood pressure > 185mmHg or diastolic blood pressure > 110mmHg, and oral antihypertensive drugs cannot control it; 8. Genetic or acquired bleeding diathesis, deficiency of anticoagulant factors, or use of anticoagulant medication with an International Normalized Ratio (INR) > 3; 9. Blood glucose < 2.8mmol/L (50mg/dl) or >22.2mmol/L (400mg/dl),platelets <100*10^9/L or Hct<25%; 10. History of bleeding within the past month; 11. Any end-stage disease with expected survival less than 6 months; 12. Anticipated inability to complete follow-up; 13. Intracranial aneurysm, arteriovenous malformation; 14. Presence of intracranial tumors with mass effect on imaging (excluding small meningiomas); 15. Brain herniation causing midline shift; 16. Presence of severe complications that may affect patient prognosis and follow-up (cancer, severe heart failure, renal failure).
