研究目的：

本课题采用随机、对照的研究方式，以确诊为 2 型糖尿病且符合肌少症诊断标准的患者为研究对象，采用以“脾胃转枢”理论为指导的电针疗法，并设立普通电针组与之对照，观察“脾胃转枢”理论指导下的电针疗法对 2 型糖尿病肌少症的临床疗效及对肌肉因子-血清鸢尾素（Irisin）的影响，为中医非药物疗法治疗 2 型糖尿病肌少症提供客观依据。  

Objectives of Study：

This topic adopts a randomized and controlled research method, taking patients diagnosed with type 2 diabetes and meeting the diagnostic criteria for sarcopenia as the research object, adopting electroacupuncture therapy guided by the theory of "spleen stomach pivot", and setting up a common electroacupuncture group to compare with it, to observe the clinical efficacy of electroacupuncture therapy under the guidance of the theory of "spleen stomach pivot" on sarcopenia of type 2 diabetes and its impact on muscle factor serum irisin, so as to provide an objective basis for Chinese medicine non drug therapy on sarcopenia of type 2 diabetes.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

符合下述 5 项标准的患者，即可纳入本项研究中： （1）符合上述西医及中医诊断标准； （2）已经稳定接受现有降糖方案 6 周以上，但血糖控制仍未达标，满足以下情况者：7%≤糖化血红蛋白（HbA1c）≤8.5%，且空腹血糖（FBG）≤10.0mmol/L； （3）35 周岁≤年龄≤85 周岁者，男女不限； （4）意识清楚，无运动及认知障碍，且能正常交流，具备独立判断力，愿意配合治疗方案及正常完成随访者； （5）自愿参与本研究并且签署知情同意书者。

Inclusion criteria

Patients who meet the following 5 criteria can be included in this study: (1) Meets the diagnostic criteria for Western and Traditional Chinese Medicine mentioned above; (2) Those who have stably accepted the existing hypoglycemic regimen for more than 6 weeks, but have not yet met the blood glucose control standards, and meet the following conditions:7% ≤ HbA1c ≤ 8.5%, and fasting blood glucose (FBG) ≤ 10.0mmol/L; (3) 35 years old ≤ Age ≤ 85 years old, no gender limit; (4) Clear awareness, no motor or cognitive impairments, able to communicate normally, possessing independent judgment, willing to cooperate with treatment Plan and those who have completed normal follow-up; (5) Those who voluntarily participate in this study and sign an informed consent form

排除标准：

凡是符合下述任何 1 项标准的患者，均应排除在本研究以外： （1）近 6 周内调整过降糖治疗方案者； （2）合并冠状动脉粥样硬化性心脏病、高血压性心脏病、肺源性心脏病、心力衰竭等心脏病、肝肾功能不全、免疫系统性疾病、慢性病急性发作期、传染病者，身体内植入金属物者； （3）近 1 月内有糖尿病酮症酸中毒等急性代谢紊乱及合并严重感染者； （4）伴发肿瘤疾病，且近半年内已接受放化疗者； （5）妊娠期、哺乳期妇女； （6）合并精神疾病或有精神异常，语言交流障碍者，无自主行为能力者； （7）研究开始前 3 个月内进行过任何有关于肌少症的治疗者； （8）听力障碍患者； （9）不配合研究者完成随访者； （10）近半年内已经参加或正在参加其他项目研究者。

Exclusion criteria：

Patients who meet any of the following criteria should be excluded from this study: (1) Those who have adjusted their hypoglycemic treatment plan within the past 6 weeks; (2) Patients with coronary atherosclerotic heart disease, hypertensive heart disease, pulmonary heart disease, heart failure and other heart diseases, liver and kidney dysfunction, immune system diseases, chronic diseases in acute attack, infectious diseases, and metal implants in the body; (3) Acute metabolic disorder such as diabetes ketoacidosis and severe infection in the last month; (4) Individuals with concomitant tumor diseases who have received radiotherapy and chemotherapy within the past six months; (5) Pregnant and lactating women; (6) Individuals with concomitant mental illness or mental abnormalities, language communication disorders, and lack of autonomous behavior ability; (7) Individuals who have received any treatment for sarcopenia within the first three months prior to the start of the study; (8) Patients with hearing impairment; (9) Those who do not cooperate with researchers to complete follow-up; (10) Researchers who have participated or are currently participating in other projects within the past six months.

选穴（参考针灸治疗学新世纪第四版）：上肢：臂臑、曲池、合谷。下肢：髀关、梁丘、足三里、阳陵泉、三阴交。针刺方法：选佳健医疗牌一次性不锈钢针（直径 0.25mm；长度40mm），针刺深度 15-35mm，得气后接电子针疗仪（华佗牌，型号：SDZ-11），电针选用两组，双侧曲池接正极，双侧足三里接负极；刺激频率：断续波 2Hz/100Hz；电流强度：根据患者耐受程度不同，从 1mA 变化到最大 10mA，留针 30min。

Intervention：

Acupoint selection (refer to the fourth edition of acupuncture and moxibustion therapeutics in the new century): upper limbs: arm, Quchi, Hegu. Lower limbs: Biguan, Liangqiu, Zusanli, Yanglingquan, Sanyinjiao. Acupuncture method: Choose a disposable stainless steel needle from Jiajian Medical brand (diameter 0.25mm; length 40mm), with a needle depth of 15-35mm. After obtaining qi, connect it to an electronic needle therapy device (Huatuo brand, model: SDZ-11). Two sets of electroacupuncture are selected, with the positive pole connected to the bilateral curved pool and the negative pole connected to the bilateral Zusanli; Stimulus frequency: intermittent wave 2Hz/100Hz; Current intensity: varies from 1mA to a maximum of 10mA depending on the patient's tolerance level, with a needle retention of 30 minutes.

Intervention：

Acupoint selection: In addition to the regular electroacupuncture group, use the Zhongwan, Right Tianshu, Left Daheng, and Qihai acupoints. The acupuncture method and electroacupuncture regimen were the same as those of the regular electroacupuncture group. Course of treatment: Both groups receive treatment twice a week for a duration of 12 weeks. Both groups were given appropriate nutritional recommendations by endocrinologists based on individual circumstances.
