研究目的：

本研究旨在构建心力衰竭伴低钠血症患者易损期容量状态精准画像平台并应用，以期精准评估患者易损期容量状态，及时准确发现并提示患者容量问题，改善患者容量管理现状，降低再住院率和死亡率，节省医疗成本，为慢病数字健康护理提供借鉴。  

Objectives of Study：

The purpose of this study is to construct and apply an accurate portrait platform for the volume status of patients with heart failure and hyponatremia in the vulnerable stage.Accurately assess the capacity status of patients in the vulnerable period, timely and accurately detect and prompt patient capacity problems, and improve patient capacity management.It can reduce the rate of re-hospitalization and mortality, save medical costs, and provide reference for digital health care of chronic diseases.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）诊断符合我国 2018 年《中国心力衰竭诊断和治疗指南》患者； （2）院内血钠<135 mmol/L； （3）无精神疾患和认知障碍，能较好地表达患病期间感受及体验； （3）能够独立使用智能手机； （5）肾小球滤过率在正常围； （6）患者自愿参加此次研究，已签署知情同意书。

Inclusion criteria

(1) The diagnosis of patients is in line with the 2018 Guidelines for the Diagnosis and Treatment of Heart Failure in China; (2) Hospital blood sodium is <135 mmol/L; (3) No mental illness and cognitive impairment, and can better express the feelings and experience during the illness; (4) Ability to use smartphones independently; (5) Glomerular filtration rate is in normal circumference; (6) Patients volunteered to participate in the study with signed informed consent.

排除标准：

（1）难治性终末期心力衰竭； （2）心力衰竭合并其他系统严重疾病

Exclusion criteria：

(1) Refractory end-stage heart failure; (2) Heart failure combined with other systemic severe diseases
