研究目的：

护理的延续性被认为是护理质量的关键指标，也是精神分裂症有效管理的关键。目前的研究通常以男性患者的数据为代表。本研究探讨延续性护理对女性精神分裂症患者社会功能、药物依从性和再入院率的影响。  

Objectives of Study：

Continuity of care is considered to be a critical indicator of quality of care and key to effective management of schizophrenia. Current studies are generally represented by data on male patients. This study examines the effects of continuity of care on social functioning, medication adherence, and readmission rates in female schizophrenic patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

纳入标准:（1）符合第十版国际疾病分类(ICD-10)精神分裂症诊断标准的女性患者;（2）住院精神病患者社会功能量表(SSFPI) 在出院时得分为29-38分;（3）年龄18 ~ 40岁;（4）告知患者及家属并签署知情同意书;（5）具有小学及以上学历;（6）住院1个月~ 3个月，符合出院标准。

Inclusion criteria

Inclusion criteria: (1) Female patients with the 10th revision of the International Classification of Diseases (ICD-10) diagnostic criteria for schizophrenia; (2) Patients with inpatient Social Function of Psychosis Inpatients (Scale of Social Function of Psychosis Inpatients, SSFPI) (Juckel and Morosini 2008) scores of 29-38 at the time of discharge; (3) Age between 18-40 years; (4) Patients and family members were informed and signed the informed consent; (5) With elementary school education or above; (6) Hospitalized for 1 month to 3 months and met the discharge criteria.

排除标准：

排除标准:（1）合并严重基础疾病患者;（2）合并智力障碍或人格障碍患者;（3）患者极度抗拒、不配合，无法配合培训实施。

Exclusion criteria：

Exclusion criteria: (1) Patients with combined severe underlying diseases; (2) Patients with combined intellectual disorders or personality disorders; (3) Patients who are extremely resistant and uncooperative and unable to cooperate with the training implementation.

干预措施：

干预组患者给予常规护理，对照组患者延续护理。建立了一支由医生、护士、心理治疗师和社区工作者组成的持续护理团队，以护士长为组长，由工作经验丰富、沟通能力强的团队成员通过考核，为患者提供持续护理。小组成员共同讨论临床病例信息，制定连续性护理计划，组长负责培训、监督和质量控制。

Intervention：

Patients in the intervention group were given conventional care followed by continuous nursing in the control group. A team of continuous nursing was established: members included doctors, nurses, psychotherapists and community workers, with the head nurse as the team leader and team members with rich work experience and strong communication skills, who passed the assessment before they could provide continuity of care for patients. The members discuss clinical case information and formulate the continuity of care plan together, and the team leader is responsible for training, supervision and quality control.
