研究目的：

（1）了解先心病患儿照顾者出院准备度现状，分析其对出院准备服务的需求。 （2）构建基于IMB模型的先心病患儿家庭参与式出院准备服务护理干预方案。 （3）观察基于IMB模型的先心病患儿家庭参与式出院准备服务护理干预对先心病患儿照顾者出院准备度、出院指导质量、照顾行为的影响。 （4）探讨基于IMB模型的先心病患儿家庭参与式出院准备服务护理干预与患儿出院后生长发育达标率、并发症发生率和非计划再入院率的关系。  

Objectives of Study：

(1) To understand the status quo of discharge readiness of caregivers of children with congenital heart disease, and analyze their demand for discharge preparation services. (2) To construct nursing intervention plan of family participatory discharge preparation service for children with congenital heart disease based on IMB model. (3) To observe the nursing intervention of family participatory discharge preparation service for children with congenital heart disease based on IMB model effects of discharge readiness, discharge guidance quality and care behavior of caregivers of sick children. (4) To explore the relationship between family participatory discharge preparation service nursing intervention based on IMB model and growth and development standard rate, complication rate and unplanned readmission rate of children with congenital heart disease after discharge.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）患儿确诊为先天性心脏病； （2）患儿年龄为0～14岁； （3）符合外科手术指征，接受手术治疗； （4）患儿主要照顾者年龄≥18岁，且每周照顾患儿时间≥72小时； （5）患儿主要照顾者具有基本的阅读、理解能力，会使用智能手机等互联网设备； （6）知情同意，自愿参加本研究，并签署书面的知情同意书。

Inclusion criteria

(1) The child was diagnosed with congenital heart disease; (2) The age of the children was 0-14 years old; (3) meet surgical indications and receive surgical treatment; (4) The primary caregiver of the child is ≥18 years old, and the time of caring for the child is ≥72 hours per week; (5) The primary caregiver of the child has basic reading and comprehension skills and can use Internet devices such as smart phones; (6) Informed consent, voluntarily participate in the study, and sign a written informed consent.

排除标准：

（1）患儿病情处于病危状态； （2）患儿主要照顾者不是患儿父母； （3）主要照顾者患有严重疾病或依从性不佳不能完成照顾任务； （4）正参加其它临床试验的受试者。

Exclusion criteria：

(1) The child's condition is in a critical state; (2) The primary caregiver of the child is not the child's parents; (3) The main caregiver has a serious illness or poor compliance and can not complete the care task; (4) Participants in other clinical trials.

实施我们科现有的护理宣教，由责任护士一对一口头为您和孩子提供入院宣教、检查宣教、术前宣教、ICU 转出宣教、出院宣教并在出院 1 个月和 3 个月进行随访。

The specific contents included: (1) Information intervention: information sharing decision-making, group teaching, early warning education, online science popularization lecture hall, public account science popularization tweet; (2) Motivational intervention: motivational interviews, peer support, online nurse-patient communication meetings; ③ Behavioral skill intervention: situational simulation training, education knowledge assessment, rehabilitation log.
