探讨超声引导头皮神经区域阻滞对神经外科开颅手术的影响  

Objectives of Study：

To investigate the effect of scalp nerve block combined with dexmedetomidine on perioperative in patients underdoing acute traumatic brain injury

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

神经外科急性创伤性脑损伤入院并行开颅手术的患者，头颅CT、MRI检查包括硬膜下血肿，硬膜外血肿，脑内血肿(包含弥漫性轴索损伤和脑干损伤)，手术方式为开颅血肿清除联合去骨瓣减压术。纳入标准：年龄25~78岁；性别不限；入院时格拉斯哥昏迷量表（GCS）评分3～12分；手术切口位于单侧，手术时间＜6 h；病案资料完整。

Inclusion criteria

For patients admitted to hospital for acute traumatic brain injury and undergoing surgery, head CT and MRI examinations included subdural hematoma, epidural hematoma, and intracerebral hematoma (including diffuse axonal injury and brain stem injury). The surgical methods were craniotomy hematoma removal combined with decompressive craniotomy. Inclusion criteria: age 25-78 years old; No gender limitation; Glasgow Coma Scale (GCS) score 3-12 at admission; The incision was unilateral and the operation time was less than 6 h. Medical records are complete.

排除标准：

合并开放性颅骨缺损者；颅内肿瘤或动脉瘤者；合并其他脏器或器官严重损伤；入院时生命体征不稳；局麻药过敏史；穿刺部位感染或破损；既往存在未控制的高血压、糖尿病；心肌病或持续性心律失常。

Exclusion criteria：

Patients with open skull defect; Intracranial tumor or aneurysm; Combined with serious injury to other organs or organs; Vital signs were unstable upon admission; History of local anesthetic allergy; Puncture site infection or damage; Previous uncontrolled hypertension and diabetes; Cardiomyopathy or persistent arrhythmia.

所有患者在插管完成后行术侧超声引导下头皮神经阻滞，0.6%的甲磺酸罗哌卡因注射液（力蒙乐）分别行眶上（2ml）、滑车上（2ml）、颧颞（2ml）、耳颞（2ml）、枕大（5ml）和枕小（5ml）神经阻滞。

all patients underwent ultrasus-guided scalp nerve block after intubation, and supraorbital (2ml), supratrochlear (2ml), jugaticotemporal (2ml), auriculotemporal (2ml), large occipital (5ml) and small occipital (5ml) nerve block were administered to 0.6% ropivacaine mesylate injection (Limonole), respectively.
