研究目的：

建立高级建立高级别的循证医学证据支持的《慢性心力衰竭中西医结合诊疗方案》，并在行业内推广，提高慢性心衰中医药治疗的整体水平  

Objectives of Study：

To establish the plan of integrate interventions including traditional Chinese medicine in treating chronic heart failure

药物成份或治疗方案详述：

经过诊疗方案培训的医生严格按照优化形成的心衰诊疗方案实施具体诊疗活动，疗程24周。 

Description for medicine or protocol of treatment in detail：

A doctor medical training in strict accordance with the optimization of diagnosis and treatment of heart failure diagnosis and plan implementation of specific treatment 24 weeks 

纳入标准：

①符合慢性心力衰竭的中西医诊断标准
②年龄40－85岁；
③NYHA功能分级II-IV级

Inclusion criteria

1. Meet the diagnosis of chronic heart failure;
2. Aged from 40 to 85;
3. NYHA cardiac functional classification: II-IV.

排除标准：

①急性心肌梗死、既往2周内的不稳定性心绞痛
②严重的心脏瓣膜病准备行心脏外科手术治疗
③终末期心衰
④合并肝肾和造血系统等严重性疾病

Exclusion criteria：

1. Acute myocardial infarction or unstable angina pectoris;
2. Serious heart valve disease preparation to surgery
3. End-stage heart failure;
4. With severe damage to liver, and kidney function, malignant tumors, blood, and other serious illnesses.
