印堂骨摸针刺法是否通过影响血清炎症因子改善慢性失眠认知障碍。  

Objectives of Study：

Does Indo-Bone Touch Acupuncture Improve Cognitive Impairment in Chronic Insomnia by Affecting Serum Inflammatory Factors.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合 DSM-5 失眠障碍诊断标准。 2. 年龄 18-65 岁，包含 18、65，性别不限。 3. 病程符合慢性失眠障碍（病程≥3 个月）。 4. 8≤失眠严重指数（ISI）≤21。 5. 右利手。 6. 签署知情同意书。

Inclusion criteria

1. Meet the DSM-5 diagnostic criteria for insomnia disorder. 2. Age 18-65 years old, including 18 and 65 years old, and gender is not limited. 3. Duration of the disease is consistent with chronic insomnia disorder (duration of the disease ≥ 3 months). 4. 8≤Insomnia Severity Index (ISI) ≤21. 5. Right sharp hand. 6. Signed informed consent.

排除标准：

1. 其他精神疾病（抑郁症、焦虑症、精分、双相等）、躯体疾病（疼痛、脑卒中、冠心病、肿瘤等）等伴发的失眠。 2. 合并严重的抑郁、焦虑，或在治疗过程中出现严重的焦虑抑郁、有自杀倾向者。 3. 睡眠卫生不良引起的失眠。 4. 时差、倒班等引起的失眠。 5. 睡眠呼吸暂停高风险者（Stop Bang 问卷≥3 项阳性判为高风险）。 6. 患有严重或不稳定、未控制良好的慢性疾病者，如高血压、肝肾功损害等。 7. 酒精、药物等物质依赖或滥用。 8. 有中枢神经系统疾病如癫痫、脑瘤、血管瘤、脑炎、急性脑损伤等病史。 9. 妊娠或哺乳期妇女、近期计划怀孕者。 10. 入组前 2 周内规律服用任何影响中枢神经系统或睡眠的药物，如助眠药、抗抑郁 药、抗精神类药、抗癫痫药等。 11. 入组前 1 月内已规律接受过针刺、TMS、TDCS、ECT 等治疗。 12. 严重视听障碍不能配合完成研究者。

Exclusion criteria：

1. Insomnia associated with other mental diseases (depression, anxiety, semen, biphasia, etc.), somatic diseases (pain, stroke, coronary heart disease, tumor, etc.). 2. Combined with severe depression, anxiety, or in the course of treatment, severe anxiety and depression, suicidal tendencies. 3. Insomnia caused by poor sleep hygiene. 4. Insomnia caused by jet lag, shift work, etc. 5. People with high risk of sleep apnea (≥3 positive items in the Stop Bang questionnaire are considered high risk). 6. People with serious or unstable chronic diseases that are not well controlled, such as high blood pressure, liver and kidney damage, and so on. 7. Alcohol, drug and other substance dependence or abuse. 8. History of central nervous system diseases such as epilepsy, brain tumor, hemangioma, encephalitis, acute brain injury. 9. Pregnant or lactating women, those who plan to become pregnant in the near future. Regular use of any medication that affects the central nervous system or sleep within 2 weeks prior to enrollment, such as sleep aids, antidepressants, and other medications. (e.g., sleep aids, antidepressants, antipsychotics, antiepileptics, etc.). 11. The applicant has received acupuncture, TMS, tDCS, ECT, etc. regularly within 1 month before enrollment. 12. Those with severe visual or hearing impairment who cannot cooperate with the study.

印堂骨膜针刺，1 次/日，共计 10 日。取穴印堂（GV28）；在额部，当两眉毛内侧端中央凹陷处。参照国家技术监督局发布的国家标准：《腧穴名称与定位》（GB/T12346-2006）。研究期间不增加腧穴。印堂穴采用提捏进针法，针体于皮肤呈 12-30°（以 针感为度），斜刺向下刺入印堂穴下 20-30mm，至骨膜中，使患者两眉头之间至鼻部有重、胀针感。留针 30 分钟。

Intervention：

Indo periosteal needling, 1x/day for a total of 10 days. The acupoint Yin Tang (GV28) is located on the forehead, in the central depression at the medial end of the two eyebrows. Reference was made to the national standard issued by the State Bureau of Technical Supervision: "Names and Positioning of Acupoints" (GB/T12346-2006). No additional acupoints were added during the study period. The Yintang acupoint was inserted by the method of lifting and pinching, with the needle body at 12-30° to the skin (in terms of needle sensation), and stabbing diagonally downward into the Yintang acupoint 20-30 mm below the periosteum, so as to make the patient feel a heavy, distended needle sensation between the two eyebrows and the nose. Leave the needle in place for 30 minutes.

予失眠症认知行为疗法（CBT-I）中的睡眠卫生教育和认知治疗。在第一次治疗时，由治疗师给患者发放一份《睡眠卫生教育+认知治疗》宣教册，由治疗师进行约 30 分钟的宣教与沟通，尽可能让患者理解宣教内容，并要求其在研究期间尝试按照宣教内容执行。

Intervention：

Sleep hygiene education and cognitive therapy in cognitive behavioral therapy for insomnia (CBT-I) were given. At the first session, the therapist gave the patient a copy of the educational booklet "Sleep Hygiene Education + Cognitive Therapy", and the therapist conducted a 30-minute teaching and communication session to make the patient understand the content as much as possible, and asked him/her to try to follow the content of the educational booklet during the study period.
