研究目的：

全生命周期健康管理是新时期医改重大命题，然而推行却陷入居民主动性与依从性低的困境。课题组前期凝练科学问题发现：“健康主动性”缺乏科学定义，以及缺乏主动健康有效的评估指标及工具。医务人员作为健康守护者，其健康状况和健康主动性尤为引起注意。因此本项目拟构建自然人群主动健康队列，聚焦医务人员，通过收集主观问卷数据研发健康主动性量表，结合居民健康档案、健康素养客观数据构建主动指数模型；在科学测量驱动健康发展的理念指导下，采用医疗级智能手表设备收集生理及健康行为等客观数据，联合体检指标、健康问卷寻找亚健康高灵敏预测指标，建立亚健康指数模型；最终两者加权结合，形成主动健康指数模型。通过真实世界就医轨迹（健康结局）对模型不断修正优化，从而实现医务人员精准主动健康评估，对于增进医务人员主动性和健康水平，推动全生命周期健康管理具有重要意义。  

Objectives of Study：

Whole lifecycle health management is a major proposition in the new round of medical reform in China. However, its implementation faces the challenge of low proactivity and compliance among residents. Our research team has previously identified the scientific problem that "health proactivity" lacks a precise scientific definition, and there are no effective evaluation indicators or tools for proactive health. As guardians of health, the health status and proactivity of medical personnel are of particular concern. Therefore, this project aims to use medical-grade smart watch and establish an proactive health cohort of general population focusing on healthcare workers. By collecting subjective questionnaire data, we will develop a health proactivity scale. This will be combined with residents' health records and objective health literacy data to construct an proactivity index model. Guided by the concept of scientifically measuring and driving health development, we will use medical-grade smart watch to collect objective data on physiological and health behaviors. These will be integrated with physical examination indicators and health questionnaires to identify highly sensitive predictive indicators of suboptimal health, establishing a sub-health index model. Ultimately, the two will be weighted and combined to form an proactive health index model. The model will be continuously refined and optimized based on real-world healthcare trajectories (health outcomes), achieving precise active health assessments for medical personnel. This will enhance the proactivity and health levels of healthcare workers and promote full lifecycle health management.

纳入标准：

①在职医务人员 ②自愿参与本研究并签署知情同意书； ③男性18-60岁；女性18-55岁 ④当前未患病或处于疾病稳定期； ⑤能够7×24 h规范佩戴医疗级智能手表（除充电、洗浴等特殊情况）； ⑥能够配合完成基线及随访调查。

Inclusion criteria

①Healthcare professionals, including clinical staff and administrative personnel working in medical institutions; ②Willingness to participate in the study and signed an informed consent form; ③Males aged 18-60; Females aged 18-55; ④Currently not suffering from illness or in a stable disease phase ; ⑤Able to wear a medical-grade smartwatche 24/7 (except for special situations such as charging and bathing); ⑥Able to cooperate in completing baseline and follow-up surveys.

排除标准：

①不愿意配合或未签署知情同意书者； ②目前处于妊娠期或哺乳期女性； ③存在严重慢性疾病（如恶性肿瘤、终末期肾病等）； ④近三个月内有重大外科手术史。

Exclusion criteria：

①Those who are unwilling to cooperate or have not signed the informed consent form; ②Pregnant or breastfeeding women; ③Presence of serious chronic diseases (e.g., cancer, end-stage renal disease, etc.) ④Major surgery within the last three months.
