研究目的：

(1)探讨经鼻湿化快速吹气换气（THRIVE）、压控通气联合咽旁加压和压控通气对麻醉诱导后术后恶心呕吐的影响。 (2）比较三种麻醉诱导吸氧方法对术中胀气、手术间隙和术后恶心呕吐的影响，并评价三种方法的差异，以寻求合适的麻醉诱导通气方法，改善患者的预后。  

Objectives of Study：

(1) To investigate the effects of transnasal humidified rapid blowing ventilation exchange (THRIVE), pressure-controlled ventilation combined with parapharyngeal compression, and pressure-controlled ventilation on postoperative nausea and vomiting after induction of anaesthesia. (2) To compare the effects of three methods of anaesthesia-induced oxygenation on intraoperative flatulence, surgical space and postoperative nausea and vomiting, and to evaluate the differences between the three methods, so as to seek for an appropriate method of anaesthesia-induced ventilation to improve the prognosis of patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①美国麻醉医师协会ASA分级Ⅰ-Ⅱ级； ②择期全麻下行腹腔镜胆囊手术的患者； ③年龄18-60岁； ④18kg/m2≤BMI≤30kg/m2。

Inclusion criteria

①ASA classification Ⅰ-Ⅱ; ②Patients undergoing laparoscopic gallbladder surgery under elective general anaesthesia; ③Age 18-60 years old; ④18kg/m2≤BMI≤30kg/m2.

排除标准：

①合并心、肝、脑、肺等重要脏器功能障碍者； ②术前可预测的呼吸道疾病、面部病理、鼻咽部损伤或疾病； ③无法通过超声获得胃部图像； ④颈椎形态或结构异常者。

Exclusion criteria：

①People with combined dysfunction of heart, liver, brain, lungs and other important organs; ②Pre-operative predictable respiratory diseases, facial pathology, nasopharyngeal injuries or diseases; ③It is impossible to obtain gastric images by ultrasound; ④People with abnormalities of cervical vertebrae morphology or structure.

本研究为双盲、随机对照研究。用随机数字法将患者分成三组经鼻湿化快速吹气通气交换组（A组）、咽旁加压组（B组）以及压力控制通气组（C组）随机数字由一个不参与研究其它部分的人员生成，并装入不透明密封袋。

The present study is a double blind, randomised controlled study.The patients were divided into three groups using the random number method Transnasal wetted rapid blow ventilation exchange group (Group A), Parapharyngeal pressure group (Group B) and Pressure controlled ventilation group (Group C) Random numbers were generated by a person not involved in the rest of the study and were placed in an opaque Ziploc bag.

盲法：

诱导过程中使用蓝布遮挡头部，负责超声测量的研究人员A观察不到通气方式，无法得知患者分组。由一名独立的麻醉医生对患者进行术后访视，该人员同样对分组不知情。所有数据最终交给另外一个研究人员（负责进行统计分析，不参加其它实验部分）进行汇总统计。在研究过程中患者，研究人员A，随访人员以及负责统计分析的人员均不知道患者的分组。

Blinding：

A blue cloth was used to cover the head during induction of anaesthesia, and researcher A, who was responsible for the ultrasound measurements, was unable to observe the mode of ventilation and could not be informed of the grouping of the patients.Postoperative visits to the patients were conducted by an independent anaesthetist, who was similarly unaware of the groupings.All data were eventually handed over to another researcher (who was responsible for carrying out the statistical analyses and did not participate in the other parts of the experiment) for the summary statistics.The patients, researcher A, the followers and the person responsible for the statistical analysis were not aware of the grouping of the patients during the study.
