研究目的：

以随机对照临床研究方法，验证足三阴经筋拨筋法治疗腰椎间盘突出症“足三阴经筋筋急”临床亚型的疗效。  

Objectives of Study：

Using randomized controlled clinical study to verify the therapeutic effect of the Foot Three Yin Myofascial Meridian Plucking Method on the "Foot Three Yin Myofascial Meridian Tension" clinical subtype of lumbar intervertebral disc herniation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 符合腰椎间盘突出症诊断标准，腰椎间盘突出节段在 L4/5 或者 L5/S1； (2) 3≤VAS≤6； (3) 年龄 18～60 岁； (4) 愿意接受本试验治疗方法，签订知情同意书者。

Inclusion criteria

(1) Meets the diagnostic criteria for lumbar intervertebral disc herniation, affected segment is L4/5 or L5/S1; (2) 3≤VAS≤6; (3) Age range: 18-60 years old; (4) Those who are willing to accept the treatment method of this study and sign an informed consent form.

排除标准：

(1) 妊娠或哺乳期妇女等不适合 X 线影像学检查者； (2)有腰部外伤史及手术史者；有腰椎肿瘤、滑脱、骨折、腰骶部移行椎者； (3) 合并腰椎管狭窄症，或合并下肢髋、膝、踝、足等肌肉骨骼系统病症者； (4) 纤维肌痛、慢性疲劳综合征等全身广泛性肌肉筋膜疼痛者； (5) 合并有心血管、脑血管、肝肾和造血系统等严重原发性疾病，精神病者。 (6) BMI 指数大于 25 者。

Exclusion criteria：

(1) Pregnant or lactating women, and others who are not suitable for X-ray imaging examination; (2) Patients with a history of lumbar trauma or surgery; Patients with lumbar tumors, spondylolisthesis, fractures, or lumbosacral transitional vertebrae; (3) Patients with combined lumbar spinal stenosis or lower limb musculoskeletal disorders such as hip, knee, ankle, and foot; (4) Patients with generalized myofascial pain such as fibromyalgia and chronic fatigue syndrome; (5) Patients with severe primary diseases such as cardiovascular, cerebrovascular, hepatic, renal, and hematopoietic systems, as well as psychiatric disorders. (6) Individuals with a BMI index greater than 25.

干预措施：

（1）腰骶部?法、掌揉法 10 分钟，要求轻柔和缓； （2）点拨 L4、L5 棘突旁压痛点 5 次；点拨 L3 横突端部 5 次； （3）患侧腰部侧卧位旋转扳法； （4）点拨患侧环跳穴、委中穴或委阳穴各 3 次，以患者能忍受为度。

干预措施代码：

Intervention：

(1) Rolling and palm kneading at lumbosacral region for 10 minutes; (2) Pressing around the tender points beside the spinous processes of L4 and L5 for 5 times; Press and pluck the soft tissues around the end of L3 transverse prcesses for 5 times; (3) Lumbar rotating manipulation with the affected side upward; (4) Press acupoints GB29, BL40, BL39 on the affected side 3 times each, based on the patient's tolerance.

Stratified block randomization was performed based on clinical research sub centers and clinical subtypes of lumbar disc herniation (muscle tension or not of the three foot Yin myofascial meridians) for stratification. The random sequence was generated by Professor Yan Shiyan, a researcher at Beijing University of Traditional Chinese Medicine, using SAS software
