研究目的：

主要目的： 1.探讨腹部电刺激对气管切开患者的咳嗽能力的影响及作用机制； 2.探讨气道廓清技术对气管切开重症患者的气道廓清能力及拔管成功率的影响； 3.观察腹部电刺激与气道廓清技术联合作用下对重症气管切开患者的早期心肺功能恢复，拔除气管套管的时间及成功率的疗效，为气切患者改善肺功能和快速并安全的拔除套管提供思路与方法。  

Objectives of Study：

Main purpose: 1. To investigate the effect and mechanism of abdominal electrical stimulation on cough ability of patients with tracheotomy; 2. To investigate the effect of airway clearance technology on airway clearance ability and success rate of extubation in patients with severe tracheotomy; 3. To observe the effect of the combination of abdominal electrical stimulation and airway clearance technology on the early cardiopulmonary function recovery, the time and success rate of tracheal tube removal in patients with severe tracheotomy, so as to provide ideas and methods for improving lung function and removing cannula quickly and safely in patients with tracheotomy.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1:气管切开患者，包括脑外伤、脊髓损伤和脑卒中等； 2.生命体征平稳；均合并肺部感染，诊断标准依据卫生部制定的《医院感染诊断标准》； 3.带管时间≥48小时； 4.病程≤6个月，年龄18-75岁。

Inclusion criteria

1: Patients with tracheotomy, including brain trauma, spinal cord injury and stroke; 2. Stable vital signs; All patients were complicated with pulmonary infection, and the diagnostic criteria were based on the Diagnostic Criteria for Nosocomial Infection formulated by the Ministry of Health. 3. Tube time ≥48 hours; 4. Duration of disease ≤6 months, age 18-75 years.

排除标准：

1:存在肋骨骨折﹑锁骨骨折、脊柱不稳定； 2.需机械通气辅助呼吸； 3.合并肺部肿瘤、肺栓塞、肺水肿、慢阻肺、哮喘等其他呼吸系统疾病； 4.有严重骨质疏松；有严重房颤； 5.血流动力学不稳定； 6.因精神心理问题难以拔除气管套管者。

Exclusion criteria：

1: Rib fracture, clavicle fracture, spine instability; 2. Need mechanical ventilation to assist breathing; 3. Combined with lung tumor, pulmonary embolism, pulmonary edema, COPD, asthma and other respiratory diseases; 4. Severe osteoporosis; Severe atrial fibrillation; 5. Hemodynamic instability; 6. It is difficult to remove tracheal tubes due to mental and psychological problems.
