探讨中低位直肠癌手术中可崩解支架原位肠转流代替回肠保护性造口的可行性及安全性  

Objectives of Study：

To investigate the feasibility and safety of replacing ileal protective ostomy with in situ colonic bypass with disintegrating stent during middle and low rectal cancer surgery

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①. 术前经肠镜检查并取活检确诊为原发性单发直肠癌，无远处转移； ②. 肿瘤下缘距肛缘≤10cm，术前初步估计可以保留肛门，且有回肠保护性造口需求； ③. 美国东部协作肿瘤组（Eastern Cooperative Oncology Group, ECOG）体能状况评分≤2； ④. 受试者自愿参加本临床研究并签署手术知情同意书。

Inclusion criteria

①. Preoperative colonoscopy and biopsy confirmed primary single rectal cancer without distant metastasis ②. The lower margin of the tumor is less than 10cm from the anal margin, and it is preliminarily estimated that the anus can be preserved before surgery, and there is a need for protective ileal stoma ③. Eastern Cooperative Oncology Group(ECOG)performance status score ≤2 ④. The subjects voluntarily participated in the clinical study and signed the informed consent for surgery

排除标准：

排除标准： ①. 心肺功能严重损害或有其它不能耐受手术的基础疾病； ②. 同时具有其他累及直肠的疾病如：家族性大肠息肉病、活动性克罗恩病或活动性溃疡性结肠炎等； ③. 合并肠梗阻需急诊手术； ④. 妊娠妇女； ⑤. 过度肥胖，BMI>35kg/m2 ⑥. 因各种原因更改手术方式、中途退出、未完成整个治疗过程； ⑦. 拒绝签署知情同意书参加试验的患者。

Exclusion criteria：

①.Serious damage to cardiopulmonary function or other underlying diseases that cannot tolerate surgery ②.Other diseases involving the rectum, such as familial colorectal polyposis, active Crohn's disease or active ulcerative colitis. ③.Complicated with intestinal obstruction requires emergency operation ④.pregnant women ⑤.Overweight, BMI>35kg/m2 ⑥.Change the surgical method for various reasons, quit midway, and did not complete the entire treatment process ⑦.Patients who refused to sign informed consent to participate in the trial

在直肠吻合口近端结肠肠腔内植入可崩解肠道支架，通过固定于支架上的薄膜将肠内容物直接转流到肛门外，达到肠内容物不经过吻合口的目的，起到类似回肠保护性造口的作用。

A disintegrating intestinal stent was implanted in the colo-intestinal lumen at the proximal end of the rectal anastomosis, and the intestinal contents were directly diverted outside the anus through the film fixed on the stent, so as to achieve the purpose of the intestinal contents not passing through the anastomosis and play a role similar to the protective ileal stomy.
