探讨尿液中特殊蛋白的联合检测在糖尿病肾脏疾病（diabetic kidney disease, DKD）早期诊断中的应用价值

To explore the value of combined urine detection of special proteins in the early diagnosis of diabetic kidney disease

纳入标准：

选取2022年1月-2023年8月在海南医学院第二附属医院住院及门诊就诊患者，符合ADA2021年制定的糖尿病诊断标准确诊为 T2DM约150例。与患者沟通，争取患者同意，留取相应的尿液样本。根据 ACR分为正常白蛋白尿（normal albuminuria, NA）组 50 例，ACR ＜30 mg/24h；微量蛋白尿（microalbuminuria, MA）组 50 例，ACR 30-300 mg/24h；显性蛋白尿组 50 例，ACR ＞300mg/24h。

Inclusion criteria

A total of 150 inpatients and outpatients in the Second Affiliated Hospital of Hainan Medical University from January 2022 to August 2023 were selected and diagnosed with T2DM according to the ADA diagnostic criteria in 2021. The patients were communicated with to obtain their consent, and corresponding urine samples were collected. According to ACR, 50 patients were divided into normal albuminuria (NA) group, with ACR < 30 mg/24h; 50 cases of microalbuminuria (MA) group, ACR 30-300 mg/24h; 50 cases of dominant albuminuria group, ACR > 300mg/24h.

排除标准：

（1） 1型及其他类型糖尿病、妊娠期糖尿病、急性高血糖紊乱者；
（2） 合并急慢性肝脏疾病者、原发性高血压；
（3） 合并甲状腺功能异常及甲状腺癌等甲状腺疾病者；
（4） 合并泌尿系结石、肉眼血尿、大量蛋白尿、急性肾损伤及终末期肾病者；
（5） 急慢性心功能不全、房颤及其他心律失常者；
（6） 急慢性感染性疾病、自身免疫病、恶性肿瘤患者；
（7） 近期行大型手术者；
（8） 贫血及其他血液系统疾病患者；
（9） 近期服用激素、抗血小板聚集等改变血流状况药物者。

Exclusion criteria：

(1) Type 1 and other types of diabetes mellitus, gestational diabetes mellitus, acute hyperglycemic disorders; (2) Patients with acute and chronic liver diseases and essential hypertension; (3) Patients with thyroid diseases such as thyroid dysfunction and thyroid cancer; (4) Patients with urinary calculi, gross hematuria, massive albuminuria, acute kidney injury and end-stage renal disease; (5) Patients with acute and chronic cardiac dysfunction, atrial fibrillation and other arrhythmias; (6) Patients with acute and chronic infectious diseases, autoimmune diseases and malignant tumors; (7) Patients who have recently undergone major operations; (8) Patients with anemia and other blood system diseases; (9) Patients who have recently taken hormones, anti-platelet aggregation and other drugs to change blood flow status.
