研究目的：

1. 基于互联网、5G、人工智能等新技术和结直肠癌风险分级筛查策略建立结直肠癌筛查早诊智能化工具，实现结直肠癌预防和筛查的全流程管理；2. 基于智能化工具的结直肠癌筛查新路径，开展全国多中心结直肠癌筛查真实世界研究，提高人群筛查效果和效率。  

Objectives of Study：

1. Based on the Internet, 5G and artificial intelligence and risk-adapted screening strategies, to establish an intelligent tool for screening and early diagnosis of colorectal cancer, and realize the whole-process management of colorectal cancer prevention and screening; 2. Based on the novel path of colorectal cancer screening with the intelligent tool, to conduct a national multi-center real-world study to improve the efficacy and efficiency of colorectal cancer screening.

药物成份或治疗方案详述：

所有入组受试者接受基于生活方式的结直肠癌风险评估，若风险评估为高风险，首先推荐其接受结肠镜检查，备选为粪便免疫化学检测（FIT）；若风险评估为中低风险，则推荐其接受粪便免疫化学检测（FIT）。所有粪便免疫化学检测阳性者推荐接受诊断性结肠镜检查。 

Description for medicine or protocol of treatment in detail：

All eligible participants will be conducted risk assessment for colorectal cancer based on an established risk prediction score; for participants of high-risk for colorectal cancer, colonoscopy is recommended first, and fecal immunochemical test(FIT) is an alternative; for participants with a low or intermediate risk, fecal immunochemical test(FIT) is recommended; diagnostic colonoscopy is recommended for all patients with positive FIT results. 

纳入标准：

1.本市户籍常驻人口 2.入组时实足年龄在45-74岁 3.愿意且能够签署知情同意书

Inclusion criteria

1. Habitant of the study region; 2. Aged 45-74 years old 3. Able to provide informed consent.

排除标准：

1. 结直肠癌病史 2. 既往进行过结肠切除手术 3. 正在接受任何癌症相关的治疗（非黑色素皮肤癌除外） 4. 近5年内接受过结肠镜、纤维乙状结肠镜、CT仿真结肠镜检查、气钡双对比造影检查。 5. 近1年内接受过粪便潜血或者粪便DNA检测 6. 有症状的下消化道疾病提示需要进行结肠镜进行确诊，包括：1)在过去6个月中有长时间的直肠出血;2)有医学诊断的缺铁性贫血;3)在过去6个月中有记录的不明原因的体重下降（超过10%基础体重） 7. 有其他严重共患疾病导致筛查获益降低或增大结肠镜检查的风险（包括严重的肺部疾病，晚期肾病，晚期肝病、严重的心衰、近期诊断为除黑色素皮肤癌外的其他癌症）

Exclusion criteria：

1. Prior history of colorectal cancer 2. Prior history of colonic resection 3. Undertaking any kind of cancer related therapy (Except for non-melanoma skin cancer) 4. Prior colonic examination, including colonoscopy, flexible sigmoidoscopy, CT colonograpy and Barium enema within five years 5. Prior history of fecal occult blood test and fecal DNA test within 1 year 6. Symptoms of lower gastrointestinal tract disease warranting colonoscopic evaluation, including:1)More than one episode of rectal bleeding within the past 6 months; 2) Documented iron deficiency anemia; 3)Significant documented unintentional weight loss (>10% of baseline weight) over 6 months 7.Significant comorbidity that would preclude benefit from screening or pose significant risk for the performance of colonoscopy (e.g. severe lung disease, end-stage renal disease, end-stage liver disease, severe heart failure, recent diagnosis of cancer (with the exception of non-melanoma skin cancer))
