研究目的：

应用近红外光谱技术 ( near-infrared spectroscopy，NIRS)对游离皮瓣进行术后72h组织氧饱和度连续监测，分析正常时和血管危象时游离皮瓣组织氧饱和度均值，通过ROC曲线下面积计算游离皮瓣血管危象阈值，多因素logistic回归调整潜在的混杂因素，评估术后72h游离皮瓣组织氧饱和度低于血管危象阈值与术后血管危象发生的相关性。以期探索游离皮瓣术后血管危象组织氧饱和度监测阈值及术后血管危象早期诊断的新路径，为术后血管危象的早发现、早诊断、早处理提供新的思路和监测模式，进一步提高游离皮瓣成活率。  

Objectives of Study：

Near infrared spectroscopy (NIRS) technology was applied to continuously monitor the tissue oxygen saturation of free skin flaps 72 hours after surgery. The mean oxygen saturation of free skin flap tissue was analyzed during normal and vascular crises. The vascular risk threshold of free skin flap was calculated based on the area under the ROC curve. Multivariate logistic regression was used to adjust for potential confounding factors, and the correlation between postoperative oxygen saturation of free skin flap tissue below the vascular crisis threshold and postoperative vascular crisis was evaluated. In order to explore the threshold for monitoring tissue oxygen saturation and a new path for early diagnosis of postoperative vascular crisis in free flap surgery, provide new ideas and monitoring modes for early detection, diagnosis, and treatment of postoperative vascular crisis, and further improve the survival rate of free flap.

纳入标准：

性别不限，年龄 18～64岁，ASAⅠ或Ⅱ级，监测区域皮肤无损伤、红肿、结节等，体温正常。

Inclusion criteria

Patients regardless of gender, aged 18-64 years, ASA grade I or II, with no skin damage, redness, swelling, nodules, etc. in the monitoring area, and normal body temperature.

排除标准：

孕产妇及哺乳期妇女，肥胖( BMI≥28 kg /m2 )，既往患有周围血管疾病、神经系统疾病者、有严重凝血功能障碍者，患处不能放置St02电极，患者拒绝参加研究或无法签署知情同意书，手术中供区血管变异无法完成手术患者，更改手术方式为非游离皮瓣手术患者。

Exclusion criteria：

Patients should be excluded from Pregnant and lactating women; Obesity (BMI ≥ 28 kg/m2);, Individuals who have previously suffered from peripheral vascular diseases, neurological disorders, or severe coagulation dysfunction; St02 electrodes should not be placed in the affected area; Patients refuse to participate in the study or are unable to sign informed consent forms; Patients with vascular mutations in the donor area during surgery who are unable to complete the surgery; Change the surgical method to non free flap surgery for patients.
