研究目的：

探索构建“3+2+1”医疗照护过渡期的连续性药学服务模式，探索基层药师慢病药学服务临床路径，为药学联合体的建立提供实践经验；实现药学服务下沉，提升社区药师慢病管理能力。  

Objectives of Study：

Explore the construction of a "3+2+1" continuity pharmacy service model for the transition period of medical care, explore the clinical pathway of chronic disease pharmacy service for primary pharmacists, and provide practical experience for the establishment of pharmacy consortiums; realize the downward sinking of pharmacy services, and enhance the capacity of community pharmacists in chronic disease management.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①入院诊断 “冠状动脉粥样硬化性心脏病”；②入院时至少有1个药物相关问题（drug related problems，DRPs）；③年龄在18~80周岁；④签署知情同意书。

Inclusion criteria

① Admission diagnosis of "coronary atherosclerotic heart disease"; ②at least one drug-related problem (DRPs) at the time of admission; ③ Age 18~80 years old; ④ Sign the informed consent form.

排除标准：

①合并严重肝、肾功能异常患者；②恶性肿瘤患者；③意识、精神和交流障碍者；④未能签署知情同意者。

Exclusion criteria：

①Patients with combined severe hepatic and renal function abnormalities; ②Patients with malignant tumors; ③People with consciousness, mental and communication disorders; ④People who failed to sign the informed consent.
