研究目的：

通过比较传统腹腔镜左半结肠及直肠癌根治术与吲哚菁绿荧光成像技术指导的左半结肠癌根治术及直肠癌根治术中的临床疗效及吻合口瘘发生率差异。进一步研究吲哚吲哚菁绿荧光成像技术辅助手术的临床意义。  

Objectives of Study：

This study aimed to evaluate the superiority and clinical implications of indocyanine green (ICG) fluorescence imaging for in anastomotic vascular perfusion assessment and rate of anastomosis leakage in laparoscopic left-sided colorectal cancer surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1).确诊时年龄为18-70岁； (2).术前或术后活检病理诊断为结直肠腺癌； (3).接受择期腹腔镜左半结肠癌根治术及直肠癌根治术，并计划行结肠-直肠或结肠-肛管吻合； (4).临床分期 TNM Ⅰ～Ⅲ 期：cT1～4N0～2M0 （AJCC- 8 Version）。

Inclusion criteria

(1) Range 18-80 years of age, (2) preoperative pathological diagnosis of colorectal cancer; (3) undergoing elective laparoscopic left colon or rectal cancer resection with one-stage anastomosis; and (4) clinical stage TNM stages I-III: cT1-4, N0-2, M0 (AJCC - 8 version).

排除标准：

(1)对 ICG 或碘过敏；(2)合并严重肠梗阻、肠穿孔、肠出血等需急诊手术的患者；(3)肿瘤累及邻近器官需要联合脏器切除者；(4)肿瘤复发或远处转移者；(5)合并结直肠多发肿瘤者；(6)肿瘤距齿状线<5cm 需行超低位切除患者；(7)ASA分级大于Ⅲ级；(8)合并妊娠的患者；(9)拟行预防性造口的患者；(10)合并炎症性肠病患者；(11)肝功能障碍，终末期肝病模型>12分 ; (12)肾功能障碍(肌酐≥2.0 mg/dL)。

Exclusion criteria：

(1)Allergy to ICG or iodine, (2)Patients with severe ileus, intestinal perforation, intestinal bleeding requiring emergency surgery, (3)Patients with tumor involvement of adjacent organs requiring combined resection, (4)Patients with distant metastases or tumor recurrences, (5)Patients with multiple colorectal tumors, (6)Patients with tumors < 5 cm from the dentate line requiring ultralow resection, (7)ASA grade greater than III, (8)Patients with pregnancies, (9)Patients with unplanned prophylactic ostomy during surgery, (10)Patients with concomitant inflammatory bowel disease, (11)Patients with combined liver dysfunction, model for end-stage liver disease > 12 points.(12)Patients with renal dysfunction (creatinine ≥2.0 mg/dL).
