研究目的：

本课题拟前瞻性录入免疫检查点抑制剂（ICI）心肌炎患者，应用心内膜心肌活检（EMB）技术，诊断ICI心肌炎患者心肌组织的病理学及临床特征的相关性，并对所有ICI心肌炎患者进行1年随访监测预后，从发病机制、临床特征以及预后危险分层方面围绕以下目标进行研究： (1)基于ICI心肌炎患者的心肌组织学，探究ICI心肌炎的组织病理学特征。 (2)通过1年ICI心肌炎患者预后随访，探讨ICI心肌炎患者的预后情况，以及基于ICI心肌炎患者的预后转归分组并探讨与不良预后相关危险因素及EMB特征结果，建立疾病的不良预后的预测模型。 (3)进一步基于ICI心肌炎患者的心肌组织，联合多组学技术，探究其发病机制与潜在的治疗靶点。  

Objectives of Study：

In this study, patients with immune checkpoint inhibitor (ICI) myocarditis were prospectively enrolled, and endocardial myocardial biopsy (EMB) was applied to diagnose the correlation between pathological and clinical characteristics of myocarditis in ICI patients, and all ICI patients with myocarditis were followed up for 1 year to monitor the prognosis. The following objectives were studied in terms of pathogenesis, clinical features, and prognostic risk stratification: (1) Explore histopathological features of ICI myocarditis based on myocardial histology of patients with ICI myocarditis. (2) To explore the prognosis of patients with ICI myocarditis through 1-year follow-up of prognosis of patients with ICI myocarditis, and to establish a prediction model of adverse prognosis based on the prognosis and outcome groups of patients with ICI myocarditis, and explore the risk factors related to adverse prognosis and EMB characteristics. (3) Further explore the pathogenesis and potential therapeutic targets based on the myocardial tissue of patients with ICI myocarditis, combined with multi-omics techniques

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄>18岁；（2）确诊心肌炎，患者必须被临床医生明确诊断为心肌炎，可以通过临床表现、心电图、心肌酶学指标、心脏超声等进行诊断；（3）接受免疫检查点抑制剂（ICI）治疗：患者必须接受免疫检查点抑制剂（ICI）治疗，可以是作为单药治疗或与其他药物联合使用；（4）能够理解研究目的，自愿参加研究并签署知情同意书。

Inclusion criteria

(1) Age >18 years old; (2) Confirmed myocarditis, the patient must be clearly diagnosed as myocarditis by a clinician, which can be diagnosed by clinical manifestations, electrocardiogram, myocardial enzyme indicators, cardiac ultrasound, etc.; (3) Receiving immune checkpoint inhibitors (ICI) : Patients must be treated with ICI, either as a single drug or in combination with other drugs; (4) Be able to understand the purpose of the research, voluntarily participate in the research and sign the informed consent

排除标准：

（1）具有其他已知心脏疾病，如冠心病、心肌病等；（2）具有其他已知心脏病因，如感染性心肌炎、药物相关性心肌炎等；（3）已接受过心脏移植或器械辅助治疗；（4）具有严重的心功能不全，NYHA分级为III-IV级；（5）孕妇或哺乳期妇女；（6）患有严重的全身性疾病，如晚期癌症、肾功能衰竭等，影响生存期预后；（7）存在心肌活检的禁忌；（8）有严重的认知或沟通障碍，无法配合研究要求；（9）近3个月内受试者参加过或正在参加其他任何器械或药物的临床试验；（10）研究者认为受试者存在其他不适宜进行临床试验的情况。

Exclusion criteria：

(1) have other known heart diseases, such as coronary heart disease, cardiomyopathy, etc.; (2) have other known causes of heart disease, such as infectious myocarditis, drug-related myocarditis, etc.; (3) have received a heart transplant or instrumental assisted treatment; (4) Severe cardiac insufficiency, NYHA grade III-IV; (5) Pregnant or lactating women; (6) Suffering from serious systemic diseases, such as advanced cancer, renal failure, etc., affecting the survival prognosis; (7) There are contraindications to myocardial biopsy; (8) Have severe cognitive or communication difficulties, unable to cooperate with research requirements; (9) The subject has participated or is participating in a clinical trial of any other device or drug within the last 3 months; (10) The investigator considers that the subjects have other conditions that are not suitable for clinical trials
