研究目的：

构建机械通气患者早期肠内营养中西医结合管理方案，探讨方案的应用效果，以改善患者的营养状况，促进护理质量的持续改进。  

Objectives of Study：

Develop an integrated Chinese and Western medicine approach for early enteral nutrition in mechanically ventilated patients, investigate the efficacy of the approach in enhancing patients' nutritional status, and enhance the ongoing enhancement of nursing quality.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.入住ICU病房满24小时； 2.年龄≥18周岁； 3.营养风险筛查≥3 分； 4.留置胃管或空肠营养管，需肠内营养支持治疗； 5.研究期间发生严重呼吸衰竭进行气管插管机械通气治疗，机械通气时间≥72 h； 6.平均动脉压≥65 mmHg。

Inclusion criteria

1. ICU stay for a minimum of 24 hours; 2. Age of 18 years or above; 3. Nutritional risk screening score ≥ 3; 4. Needs gastric or enteral nutrition tube placement for enteral nutrition support therapy; 5. Experienced severe respiratory failure in the study, underwent mechanical ventilation with endotracheal intubation, and had mechanical ventilation lasting ≥ 72 hours; 6. Mean arterial pressure of ≥ 65 mmHg.

排除标准：

1.存在活动性上消化道出血、肠梗阻等肠内营养禁忌证； 2.颅脑损伤病人； 3.腹部手术后病人； 4.因不同种族及文化原因对营养有特殊要求者； 5.不愿参加该研究的病人。

Exclusion criteria：

1. Patients with active gastrointestinal bleeding, intestinal obstruction, or other contraindications to enteral nutrition. 2. Patients with head or brain injuries. 3. Patients post-abdominal surgery. 4. Patients with unique nutritional requirements based on different races and cultural considerations. 5. Patients who decline to participate in the study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确，清晰地录入病例报告表。录入采用相应的数据库系统双人双机录入，之后对数据进行两遍比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Researchers shall enter data into the case report forms timely, completely, accurately, and clearly, based on the original observation records of the subjects. Data entry is performed using a dual data entry system by two individuals on separate computers, followed by two rounds of data verification. Electronic data files are categorized and stored, with multiple backups kept on different disks or recording media to ensure proper preservation and prevent damage.
