衰弱。是老年人生理储备下降导致机体易损性增加、抗应激能力减退的非特异性状态,是健康到死亡的中间状态,可分为躯体衰弱与认知衰弱。主要临床症状有疲乏、非有意的体重减轻、步速下降、握力下降、活动减少等。衰弱是导致老年人跌倒、失能、共病、死亡等的主要因素,严重威胁人口健康,加重国家医疗负担和阻碍社会经济发展。  

Target disease:

Frailty. It is a non-specific state caused by the decline of the physiological reserve of the elderly. It is an intermediate state from health to death, which can be divided into physical frailty and cognitive frailty. 本研究拟对老年衰弱患者进行营养干预,观察营养干预对老年衰弱的改善效果,建立衰弱症数字化营养干预的技术框架,本研究结果可能为老年衰弱的治疗和管理提供一种可行性方案。  

Objectives of Study:

This study intends to conduct nutritional intervention in elderly frailty patients, observe the improvement effect of nutritional intervention on geriatric frailty, and establish the technical framework for digital nutritional intervention in frailty. The results of this study may provide a feasible program for the treatment and management of frailty in old age.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄≥60岁;
2.衰弱筛查阳性患者;
3.根据MNA-SF评估存在营养不良风险或营养不良(评分≤11分);

Inclusion criteria

1.Age: 60 years old;
2.Patients with positive frailty screening;
3.Risk of malnutrition or malnutrition as assessed by MNA-SF (score 11).

排除标准:

1.长期卧床或口服食物存在困难者;
2.对口服膳食补充剂成分过敏或不适应者;
3.患帕金森病、癫痫和其他神经系统疾病者;
4.恶性肿瘤终末期、严重痴呆不能交流、急性心肌缺血或心力衰竭发作、纽约心脏病学会新功能分级IV级、急性上消化道出血、严重肝、肾功能不全;
5.干预前1个月内补充乳清蛋白粉或其他口服全营养补充剂或正在参与其他营养干预项目(如产品成分含糖、脂肪量、盐分过高,需排除糖尿病、高脂血症、高血压等);

Exclusion criteria:

1.Patients with chronic bedridden or oral food difficulties;
2.Allergic to or inadaptive to oral dietary supplements;
3.For Parkinson's disease, epilepsy and other neurological diseases;
4.End stage of malignancy, severe dementia inability to communicate, acute myocardial ischemia or heart failure episode, New York Society of Cardiology New Functional Classification grade IV, acute upper gastrointestinal bleeding, severe hepatic and renal insufficiency;
5.Supplement whey protein powder or other oral total nutritional supplements within 1 month before the intervention or are participating in other nutritional intervention programs (such as product components containing sugar, fat content, excessive salt, excluding diabetes, hyperlipidemia, hypertension, etc.).

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

该研究需采集变量繁多,涉及较多的量表问卷,单纯人工采集测算费时费力且受试者依从性难以保证。利用华东医院老来宝小程序,采集CRF及受试者电子病历,对老年人群进行老年疾病早期筛查和评估,调查疾病史、用药史、生活史,形成营养健康评估,结合营养干预方向,综合考虑营养补充剂的功效、补充对象的疾病和用药史、生活史情况,智能化生成营养补充方案,能及时监督受试者营养补充及锻炼,增加受试者依从性。对营养补充方案有效的评价,在营养补充对象的自我感觉的基础上,采用健康监测和在体数据等客观评估手段,促进方案有效实施。老来宝小程序里的健康宣教专家讲堂、营养课堂以及运动锻炼视频等,能提供受试者所需的营养及运动指导,保证营养干预的效果与运动干预、健康科普宣教相结合地同步推进。

In this study, many variables need to be collected, and many scale questionnaires are involved. Simple manual collection and measurement are time-consuming and laborious, and it is difficult to guarantee the subject compliance. Using east China hospital "lao laibao" small program, the elderly early disease screening and evaluation, disease history, drug history, life history, nutrition health assessment, combined with nutrition intervention direction, considering the effect of nutritional supplements, supplement object disease and drug history, life history, intelligent generation nutrition supplement scheme, can timely supervise the subjects nutrition supplement and exercise, increase subject compliance. The effective evaluation of the nutritional supplement program, based on the self-perception of the nutritional supplement object, adopts objective evaluation means, such as health monitoring and in-body data, to promote the effective implementation of the program. The health education expert lecture hall, nutrition class and exercise video in the small program can provide the nutrition and exercise guidance needed by the subjects, and ensure that the effect of nutrition intervention is promoted simultaneously with exercise intervention and health science education.
