研究目的：

比较胰岛素强化治疗和胰岛素常规治疗对严重烧伤患者的疗效。  

Objectives of Study：

Compare the effects of intensive insulin therapy with conventional insulin therapy in severely burned patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、年龄在18岁到60岁之间（18岁≤年龄≤60岁）；2、热力烧伤患者；3、烧伤总面积≥30%；4、伤后72小时内入院。

Inclusion criteria

1. Aged between 18 and 60;
2. Thermally injured patients;
3. Burns more than 30% of total body surface area (TBSA);
4. Admission within 72 hours after thermally injury.

排除标准：

1、非热力因素（电接触、化学、放射等）引起的烧伤患者；2、原有的非烧伤引起的严重心、肝、肾等器官功能障碍者；（烧伤前心功能Ⅲ级或Ⅳ级，有中度或严重心血管疾病的客观检查依据；总胆红素≥33μmol/L；肌酐≥171μmol/L或尿量≤500ml/24h）；3、孕期及哺乳期妇女；4、有严重复合伤不能在烧伤专科治疗的患者；5、因其他疾病使用糖皮质激素的患者；6、1型糖尿病患者、存在酮症酸中毒或非酮症高渗性糖尿病昏迷的患者；7、使用免疫抑制剂的患者；8、预计生存时间少于24小时的患者；9、在参加本试验前1月内曾参加其他药物临床试验或正在参加其他临床试验的患者；10、吸毒或严重酗酒者；11、未签署知情同意书或不能补签知情同意书的患者。

Exclusion criteria：

1. Burned patients but non-thermally injured (electric burn, chemical injury, radical injury, et al);
2. Patients with severe cardiac, hepatic or renal dysfunction before injury (cardiac function classification, by NYHA, grade III or IV; total bilirubin >=33umol/L; creatinine>=171umol/L or urine output<=500ml/24h);
3. Pregnant or lactating women;
4. Patients would be treated in other department for severe multitrauma;
5. Patients treated with glucocorticoid for other disease;
6. Patients with type 1 diabetes mellitus, diabetic ketoacidosis or nonketotic hyperosmolar diabetic coma;
7. Patients treated with immunosuppressive agents;
8. Patients with a life expectancy less than 24 h;
9. Patients who are participating or have participated in other clinical trials within 1 month before this trial;
10. Drug addicts or heavy drinkers;
11. Patients who can not provide prior informed consent or delayed informed consent.
