研究目的：

本项目研究目标是评价刺激耳迷走神经治疗功能性便秘和糖尿病便秘的疗效。同时探讨刺激耳迷走神经治疗功能性便秘和糖尿病便秘的可能相关机制。  

Objectives of Study：

The objective of this project is to evaluate the efficacy of stimulating the auricular vagus nerve in the treatment of functional constipation and diabetic constipation. Also to investigate the possible mechanisms associated with stimulation of the auricular vagus nerve in the treatment of functional constipation and diabetic constipation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄18-75岁，性别不限。患者能与研究者进行良好沟通，并签订知情同意书。 符合慢性便秘的诊断标准：慢性便秘的诊断主要基于症状, 可借鉴罗马IV标准中功能性便秘诊断标准所述的症状和病程。 糖尿病便秘组需符合纳入标准和糖尿病诊断标准：采用2010年美国糖尿病协会发布的糖尿病诊断标准，即满足条件1-4其中1条：①糖化血红蛋白HbAlc>6.5%；②空腹血糖FPG≥7.0mmol /L。空腹定义为至少8h内无热量摄入；③口服糖耐量试验时2h血糖≥11.1mmol /L（按照WHO的标准，采用75g无水葡萄糖进行口服糖耐量试验）；④在对伴有典型的高血糖或有危象症状的患者，随机血糖≥11.1mmol/L；⑤在无明确高血糖时重复检测来证实标准①-③。

Inclusion criteria

Age 18-75 years old and gender is not limited. Patients are able to communicate well with the investigator and sign an informed consent form. Meet the diagnostic criteria for chronic constipation: the diagnosis of chronic constipation was based mainly on symptoms, which could be based on the symptoms and duration of the disease as described in the Rome IV criteria for the diagnosis of functional constipation. Diabetic constipation group should meet the inclusion criteria and diagnostic criteria of diabetes mellitus: the diagnostic criteria of diabetes mellitus issued by the American Diabetes Association in 2010 were used, i.e., one of the conditions 1-4 was met: ① glycosylated hemoglobin HbAlc>6.5%; ② fasting blood glucose FPG≥7.0mmol /L. Fasting was defined as no caloric intake for at least 8h; ③ 2h blood glucose at oral glucose tolerance test≥11.1mmol /L (oral glucose tolerance test using 75g of anhydrous glucose according to WHO standards); ④ in patients with typical hyperglycemia or with critical symptoms, random blood glucose ≥ 11.1mmol/L; ⑤ repeated testing in the absence of clear hyperglycemia to confirm criteria ①-③.

排除标准：

1）药物导致的慢性便秘，如抗抑郁药、阿片类药物、抗癫痫药物等； 2）有恶性肿瘤病史； 3）六个月内有腹部手术史（除外阑尾切除术、胆囊切除术及刨宫产等）； 4）排除肠道器质性病变； 具有严重的心、肝、肺、肾、血液、免疫或影响其生存的严重疾病者；认知功能障碍、失语、精神障碍、或可能影响患者合作，不能配合完成研究过程的疾病；过去3个月内参加过其他临床医学研究；研究者判断不适合参与本研究者；妊娠期、准备怀孕或哺乳期患者；影响电极固定的皮肤病或者对表面电极贴片过敏者；熟悉针灸治疗、取穴或经络；研究期间因病情变化或诊疗需要而采取药物、灌肠等导泻通便方法的患者；糖尿病便秘组：发现糖尿病之前即存在便秘症状者；其他任何引起便秘或神经系统疾病，如多发性硬化症、松弛症、帕金森病或脊髓损伤等。

Exclusion criteria：

1) Chronic constipation caused by medications, such as antidepressants, opioids, antiepileptic drugs, etc; 2) History of malignant tumors; 3) history of abdominal surgery within six months (except appendectomy, cholecystectomy and planarian delivery, etc.) 4) exclusion of organic lesions of the intestinal tract; Those with severe heart, liver, lung, kidney, blood, immune or serious diseases affecting their survival; cognitive dysfunction, aphasia, psychiatric disorders, or diseases that may affect the patient's cooperation in completing the study procedure; participation in other clinical medical studies within the past three months; those who are judged by the investigator to be unsuitable for participation in this study; patients during pregnancy, preparation for pregnancy or breastfeeding; skin diseases affecting electrode fixation or allergic to surface electrode patches; patients familiar with acupuncture treatment, acupuncture points or meridians; patients who take drugs, enemas and other laxative and laxative methods due to changes in condition or treatment needs during the study period; diabetic constipation group: those with constipation symptoms prior to the discovery of diabetes; any other constipation-causing or neurological diseases such as multiple sclerosis, flaccidity, Parkinson's disease or spinal cord injury.
