本研究拟通过基于学校的整群随机试验,利用由中国健康教育中心和乔治全球健康研究院共同研发的一套适用性强的减盐健康教育干预服务包,以小学生及其家庭减盐教育为核心,将其纳入现有的学校健康教育体系,在小学开展干预并实时优化,分析其可行性和有效性,为降低人群盐摄入量、减少高血压和心脑血管疾病等慢性病的发生提供干预方法和理论依据。  

Objectives of Study:

This research plans to utilize a set of highly applicable Service pack of health education intervention concerning to salt reduction jointly developed by China Health Education Center and George Global Health Institute to integrate salt reduction education of primary school students and their families with the existing school health education system, implement intervention and real-time optimization in primary schools, and analyze its feasibility and effectiveness through school-based cluster randomized trials, aiming at providing intervention methods and theoretical basis for reducing salt intake, hypertension, cardio cerebral Vascular disease and other chronic diseases.

药物成份或治疗方案详述:

本研究包括520名学生和520名成人家属。 以“小手拉大手,减盐齐步走”为口号,对学校三年级学生进行减盐教育,以期通过孩子带动全家的方式降低儿童及其家庭的盐摄入量。干预期限为1个学年。干预内容主要组成部分包括线上和线下两大部分,具体内容、形式和频次可在规定基础上,由当地教育部门、学校和班级自行计划和实施。对于个别郊县和农村学校,如果不具备实施线上部分的干预工作的客观条件,比如外出务工父母占比大、大部分家长不会使用智能手机等,可以采用课堂播放视频和老师讲解的方式进行补充和替代,但须确保8节减盐必修课程和4次减盐互动活动在全部三年级学生和家庭中得到全面落实。 线上部分 基于EduSaltS微信小程序的减盐课程:参照教育部《中小学健康教育指导纲要》中规定的教学内容,项目开发的“健盐云课堂”微信小程序每学期发布10节健康课,每周在线发布1节,每节课视频时间约5分钟,要求学生在家和家人一起观看,并完成课后10道测试题,以提升学习兴趣,并检验对本节课知识的掌握情况。健康视频课中包含8次减盐必修课(内容涉及食盐过多的危害、减盐误区和减盐实用技巧等)和12次健康选修课(例如保护视力、营养膳食、预防呼吸道传染病等),班主任老师负责组织学生及其家长使用小程序学习减盐课程,并通过教师端小程序(一款用于监测、管理和评价的微信小程序)随时获知每个学生家庭课程学习和作业完成的情况,及时督促学生和家长按时按要求进行。 微信小程序其他功能:(1)优食优选:用户通过扫描食品条形码,可以识别和对比预包装食品营养标签中的钠信息,给用户提供食盐相关提示,并推荐含盐量更低且更健康的同类产品。(2)健康知识擂台:通过即时参加1对1知识竞答,增进学生和家长对健康知识的学习兴趣,促进他们对健康知识的正确理解。(3)盐值测评:学生和家长通过称量家庭含盐调味品重量,并连续记录七天的家庭用餐情况,可准确评估出家庭及成员的食盐摄入量以及食盐的主要来源。小程序还将提供具体减盐行动建议和健康饮食建议,帮助用户采取有效和可行的方法减少食盐,健康饮食。 线下部分 减盐互动活动:结合学校现有的健康教育课、综合实践课、科学实践课、主题班会、家长会等已有课程体系,每个学期开展至少4次的减盐讲座和主题活动。班主任老师可以利用减盐网站提供的视频、海报、折页等素材和工具向学生讲解减盐知识。孩子们在课后要完成相关的家庭作业,并向家人传递减盐的知识、方法和技巧,说服在家做饭的人减少盐的使用量。 为了加强减盐的趣味性,激励学生和家长对减盐的积极性,班主任还可以组织学生和家长减盐交流会、减盐主题班会、减盐知识竞赛活动、减盐艺术创意大赛、减盐作文比赛等互动式活动。 减盐环境建设:在学校校园、食堂、教室张贴减盐海报,创建减盐校园环境;在家庭张贴减盐小海报和减盐冰箱贴;班级播放由中国健康教育中心监制的减盐系列短片;给每个家庭发放限盐勺、盐罐、减盐小贴纸、减盐折页和减盐知识手册等。 学校餐食减盐:针对学校食堂厨师开展减盐健康教育和培训,监测食堂用盐量;与提供校餐的餐饮企业开展合作,减少学生用餐的含盐量。  

Description for medicine or protocol of treatment in detail:

This study included 520 students and 520 adult family members. Our slogan is Small hands leading big hands, together lets reduce salt. The intervention includes two parts: online section and offline section. If the online section is not feasible for schools in suburban and rural areas (e.g. parents work in other cities, family adults do not have a smartphone), teachers can play videos in the class and explain to students. At least 8 salt reduction courses and 4 class actives (should be conducted in all classes of grade 3 in selected schools and their families. Online section EduSaltS WeChat program based salt reduction course: According to the Primary and Middle School Student Health Education Guidance Outline issued by the Ministry of Education in China, EduSaltS will release an online 5-minute health education course weekly and 10 classes in total per semester. The student and parents are required to learn together, and then finish 5-10 quizzes for increasing interest in learning and a better understanding of the knowledge. Eight mandatory salt reduction courses (e.g. health effects associated with excessive sodium consumption, high-salt foods) and 12 health elective courses (such as prevention of myopia and heathy diet) will be delivered. The teacher will monitor compliance using the App and remind the student and parents to finish the course on time. EduSaltS WeChat program other functions: (1) Good food choiceThe App can identify and compare nutrition information such as energy, sodium, and carbohydrates in pre-packaged foods by scanning the food barcode. It can recommend similar products that are heathier and have a lower salt content. (2) knowledge competitions: The one-on-one knowledge competition can increase the interest of students and parents in learning health knowledge and promote a better understanding of these knowledge. (3) Salt intake assessment: Students and parents can accurately assess the intake of salt and its main sources by weighing cooking salt at home and recording the dietary for seven consecutive days. The program will also provide specific recommendations on salt reduction and healthy diet, which could help App users to effectively reduce salt intake and prepare healthy diet. Offline section Interactive activities: In order to motivate students and their parents to attend salt reduction activities, interactive activities (e.g. themed class meeting and meeting with family participation) which can be integrated into the existing education curriculum, should be carried out at least 4 times per school term. Teachers can explain the salt reduction knowledge to students using materials such as videos, poster, and flyers, and assign some homework on salt reduction for students. Students are required to educate their family members, particularly the person who does the cooking, on reducing the amount of salt used at home. Salt-reduction environment building: To build salt-reduction environment in the school, such as salt-reduction posters in campus, restaurants, and classrooms, playing videos about salt reduction. To build salt-reduction environment at home by providing small poster, fridge magnets with salt reduction slogans, salt reduction leaflet, salt-restriction-spoon, salt-box, and others. School meals with low salt: Conduct health education and training about salt reduction among chefs in school cafeterias, and monitor the use of salt; Work together with school meals catering companies (outside school) to reduce salt in food.  

纳入标准:

(1)愿意接受随访;(2)减盐项目开展期间,学生没有转学计划;(3)参与调查的学生家属年龄大于18岁,小于75岁;(4)能够按要求定期反馈以及完成问卷调查和结局变量的测定

(1)儿童有相关医学疾病(如肾功能不全等)或其他原因不能参与调查或干预活动的;(2)明确不能完成随访的;(3)明确不能收集或者拒绝收集24小时尿的个人。

先运用PPS分层抽样,根据人口规模、城乡分布、学校规模、食堂设置与否、配餐情况等因素在赣州市章贡区、南康区、信丰县和于都县分别抽取8、6、4、2个学校。然后采用随机数字法整群随机化分组,将随机抽取的20个学校根据县区位置、城乡分布、学校规模与硬件设施情况、食堂设置与否、配餐情况等分层因素随机分成干预组和对照组,每组各10所学校。章贡区、南康区、信丰县和于都县分别有4、3、2、1所干预和对照学校,确保各县区干预组与对照组学校均衡可比(如均在城区/乡镇,规模相似,均有/食堂或是否统一配餐等)。

Randomization Procedure (please state who generates the random number sequence and by what method):

Firstly, PPS Stratified sampling was used to select 8, 6, 4 and 2 schools in Zhanggong District, Nankang District, Xinfeng County and Yudu County of Ganzhou City according to the population size, urban and rural distribution, school size, canteen setting, catering situation and other factors. Then using the random number method for cluster randomization, 20 randomly selected schools were randomly divided into an intervention group and a control group based on stratified factors such as county location, urban-rural distribution, school size and hardware facilities, cafeteria settings, and meal preparation, with 10 schools in each group. There are 4, 3, 2 and 1 intervention and control schools in Zhanggong District, Nankang District, Xinfeng County and Yudu County, respectively, to ensure that the intervention group and the control group in each county are balanced and comparable (for example, they are all in urban areas/towns, with similar scale, and have/canteen or unified catering).

盲法:

由于本研究干预方案执行的特殊性,必须明确告知干预组具体相关干预事宜,因此无法对干预实施者进行盲法控制,但对于资料收集者,采用盲法。资料收集者在经过严格的培训后,在不被告知干预分组的情况下,去现场收集各类资料。此外,其他控制混杂的方法主要包括:(1)核查餐饮供应机构是否存在服务于多家小学的情况,尤其是否存在同时服务于干预组和对照组的情况。(2)严格要求干预组学校工作人员切勿以任意网络媒体/社交媒体的形式进行项目相关干预或干预信息的公布,并由班主任告知学生家长同样遵循此规则。当今网络信息的传播速度快、范围广,对该情形加以控制,以免造成对照组的沾染;③在完成干预组和对照组的随机分组后,严格比对干预组和对照组学校的地理分布,避免出现个别干预组和对照组相距较近的现象,以防沾染。

Blinding:

Due to the special nature of the intervention plan implemented in this study, it is necessary to clearly inform implementers of intervention group with intervention actions. Therefore, blind control cannot be applied to the intervention implementers, but for data collectors, blind control is used. After strict training, data collectors will go to the site to collect various types of data without being informed of intervention groups. In addition, other methods for controlling confounding mainly include: (1) verifying whether the catering supply institution serves multiple primary schools, especially whether there are cases of serving both the intervention group and the control group simultaneously. (2) Strictly requiring the school staff of the intervention group not to disclose project related interventions or intervention information in the form of any online media/social media, and asking the homeroom teacher to inform students and parents to follow this rule as well. 数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

基线调查和随访阶段,调查员通过安装在手机上的电子数据采集系统(mEDC)进行基线和随访数据的采集、录入和自查。各级指定的数据管理员将对原始调查数据进行实时核查,若发现可疑数据,及时通知地方协调员,要求调查员回答和修订。他们之间的各种疑问及解答修订的过程都在mEDC上用疑问形式完成,mEDC系统会保留所有记录的痕迹。当所有数据核查无误后,数据管理员将形成核查报告,其内容包括入选/排除标准、数据完整性、逻辑一致性、完成情况等。干预期间,通过“健盐云课堂”手机微信小程序,系统可以自动采集到干预实施过程的数据,包括课程学习和任务完成情况、家庭及个人的盐摄入量和主要来源的评估结果等,以及登录和使用AppSalt的日志记录。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The data will be collected, entered, and checked through mEDC by well-trained field investigators at baseline and during follow-ups. Data managers will check the raw data in real time, and contact filed investigators if any issues exist. The investigators are required to check and correct the data if necessary. All communications will be recorded in the system. When all the data are checked, data managers will produce a data check report, including inclusion/exclusion criterial, data complement, logic check, and so on. During the intervention period, the system will automatically collect data about the study implementation, including course learning, assignments, family level and individual level salt intake and main sources, use of AppSalt diary.
