探讨踩跷疗法治疗腰椎间盘突出症的临床有效性及安全性  

Objectives of Study：

To investigate the clinical efficacy and safety of stilt therapy in the treatment of lumbar intervertebral disc herniation

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①诊断为腰椎间盘突出症的患者； ②18周岁≤年龄≤65周岁； ③受试者自愿参加并签署知情同意书； ④经CT或MRI证实为L3 /L4、L4 /L5、L5 /S1椎间盘轻度或中等突出，突出物不超过5mm或突出物压迫硬膜囊不超过30%； ⑤患者无明确手术指征，选择保守治疗。

Inclusion criteria

(1) Patients diagnosed with lumbar intervertebral disc herniation; (2) 18 years old≤ age ≤ 65 years old; (3) Subjects voluntarily participate and sign the informed consent form; (4) Mild or moderate herniation of L3/L4, L4/L5, L5/S1 intervertebral disc confirmed by CT or MRI, with the protrusion not exceeding 5mm or the protrusion compressing the dural sac not exceeding 30%; (5) The patient had no clear indication for surgery and chose conservative treatment.

排除标准：

①合并马尾综合征、脊柱骨折、骨结核、黄韧带肥厚干性坐骨神经痛、梨状肌损伤综合征、脱位、腰椎滑脱、严重骨质疏松、有钙化、突出物与神经根严重粘连、腰椎管狭窄等疾病者； ②妊娠期及哺乳期女性； ③合并存在出血倾向的造血系统疾病，肿瘤，严重心、脑、肝、肾、血液系统等重要脏器严重疾病的并发症；合并认知或精神障碍等精神心理疾病患者； ④患有风湿性、类风湿关节炎的患者； ⑤有腰背部皮肤破损、外伤等； ⑥研究者判断不适宜参加试验的其他情况。

Exclusion criteria：

(1) Patients with cauda equina syndrome, spinal fracture, bone tuberculosis, ligamentum flavum hypertrophy, dry sciatica, piriformis injury syndrome, dislocation, lumbar spondylolisthesis, severe osteoporosis, calcification, severe adhesion between protrusions and nerve roots, lumbar spinal stenosis and other diseases; (2) Pregnant and lactating females; (3) Complications of hematopoietic system diseases with bleeding tendency, tumors, serious diseases of heart, brain, liver, kidney, blood system and other important organs; Patients with mental and psychological diseases such as cognitive or psychiatric disorders; (4) Patients with rheumatoid and rheumatoid arthritis; (5) Skin damage and trauma on the lower back; (6) Other circumstances judged by the investigator to be inappropriate to participate in the trial.

患者俯卧，以九式十六脚法的前八式套路治疗后，重点再以腰椎间盘突出部位的两侧华佗夹脊穴、膀胱经1.5寸内侧线局部按揉5分钟，跷法3-5遍，颈肩部位至腰骶关节为止点滑法5遍，滑法至腰骶或骶髂关节处蹬法3次。 患者改侧卧位，健侧下肢伸直，患侧下肢屈膝屈髋80°-90°，医者采用十六脚法的点、按、揉法于环跳穴、风市穴、阳陵泉各2分钟，脚掌、脚跟交替按揉臀部肌肉5分钟。脚弓滑法于髂胫束3-5遍。 患者就坐与膝等高的方凳，医者蹲在其身后，患者向前弯腰最大限度时，医者用双手拇指推按腰部夹脊穴，同时嘱患者缓慢直起腰，按揉局部肌肉，依次推按3遍。

Intervention：

The patient is prone and treated with the first eight routines of the nine-style sixteen-foot method, and then focuses on the Huatuo chiropractic points on both sides of the lumbar intervertebral disc herniation and the bladder through the 1.5-inch medial line for 5 minutes, 3-5 times of stilt, 5 times of point sliding from the neck and shoulder to the lumbosacral joint, and 3 times of sliding to the lumbosacral or sacroiliac joint. The patient changed to a lateral decubitus position, the lower limb on the unaffected side was straightened, the lower limb on the affected side was bent at 80°-90°, and the doctor used the 16-foot method of pointing, pressing and kneading on the Huanqiao acupoint, Fengshi acupoint and Yangling Spring for 2 minutes each, and the soles of the feet and heels were alternately pressed and rubbed the buttock muscles for 5 minutes. Slide the arch of the foot on the iliotibial band 3-5 times. When the patient bends forward to the maximum, the doctor pushes the thumbs of both hands to push the waist and spine points, and at the same time asks the patient to slowly straighten up, knead the local muscles, and push 3 times in turn.

双手扶住踩跷床的横杆，控制踩踏的力量，用脚跟部位对L3、L4、L5、S1椎旁两侧1～2cm处进行垂直加压，以膝关节的屈伸动作，使身体一起一落。频率为60～100次/min，根据患者的承受能力进行力量控制和调整，踩跷的力量由轻到重。踩跷操作5～10min，期间可以休息3～4次，休息30s/次左右。
